    Abstract: Objective To investigate the effect of exercise load on apparent diffusion coefficient (ADC) and fractional anisotropy (FA) of normal lumbar intervertebral discs in magnetic resonance (MR) diffusion tensor imaging (DTI) . Methods Thirty healthy volunteers (24 males and 6 females, aged 19 to 25 years) underwent examinations with MR T2WI and DTI of the lumbar intervertebral discs before and after exercise load. Pfirrmann grading was evaluated with T2WI, and the B0 map, ADC map and FA map were reconstructed based on the DTI data to investigate the changes in ADC and FA after exercise. Results Of the 30 volunteers (150 intervertebral discs) receiving the examination, 27 with discs of Pfirrminn grade II were included for analysis. In these 27 volunteers, the average ADC and FA before exercise were (1.99 �0.18) �10-3 mm2 /s and 0.155 �0.059, respectively. After exercise, ADC was lowered significantly to (1.93 �0.17) �10-3 mm2 /s (P<0.05) and FA increased slightly to 0.1623 �0.017 (P>0.05). Conclusion DTI allows quantitatively analysis of the changes in water molecular diffusion and anisotropy of the lumbar intervertebral discs after exercise load, which can cause a decreased ADC and a increased FA value, and the change of ADC is more sensitive to exercise load.

    Abstract: Objective To determine the effect of resveratrol on constrictions of isolated human intrapulmonary arteries and its mechanisms. Methods Intrapulmonary arteries (1-1.5 mm in diameter) were dissected and cut into rings (1.8-2.0 mm in length) under microscope, and were then mounted in a Multi Myograph system. The rings were stimulated with 100 nmol/L U46619, 30 nmol/L endothelin-1, or 60 mmol/L KCl to produce sustained contraction of the intrapulmonary arteries, after which resve-ratrol was applied cumulatively. Endothelium denudation, L-NAME and indomethecin were used to investigate the effect of resveratrol on constrictions of the isolated arteries, suing DMSO as the control. Results Resveratrol induced concentration-de-pendent relaxations in endothelium-intact rings that contracted in response to stimulations with U46619, ET-1 and KCl, with pD2 of 3.82�.20, 3.84�.57, and 3.68�.27, Emax of (99.58�.83)%, 100%, and (99.65�.98)%, respectively. Treatment of the arterial rings with the eNOS inhibitor L-NAME, but not with indomethecin or endothelium denudation, obviously affected the relaxant effects of resveratrol. Conclusion Resveratrol can concentration-dependently produce relaxant effect on human intrapulmonary arteries independent of the endothelium possibly by promoting synthesis and release of NO.

  • 国产内镜吻合器在腹腔镜辅助直肠癌根治术中的应用

    Abstract: Objective To investigate the safety and feasibility of domestically made endoscopic stapling instrument in laparoscopic assisted rectal cancer resection (Dixon). Methods Sixty-four patients with rectal cancer were randomly divided into the research group (35 cases) to receive laparoscopic assisted rectal cancer resection using ENDO RLC general endoscopic linear cutter and single-use loading unit and circular staplers with staples (from REACH medical equipment co.LTD) and the control group (29 cases) to receive surgery with the corresponding products widely used (fom Johnson&Johnson Mdical Euipment C.Ltd). The clinical data of the two groups were compared. Results Satisfactory therapeutic effects were obtained in all the cases. The two groups showed no significant differences in the operative time, intraoperative anastomosis success rate or postoperative complications (anastomotic bleeding, leakage, or stricture) between the two groups (P>0.05), but the average cost of endoscopic stapling instrument was significantly lower in the research group (6604.31�99.95 vs 7822.28�76.98 RMB Yuan, P<0.05). Conclusion The domestic endoscopic stapling instrument is safe, effective and less costly for laparoscopic assisted rectal cancer resection.

  • 连续性血液净化治疗体外循环堵管风险积分模型的构建

    Abstract: Objective To establish a prognostic model for predicting extracorporeal circulation clotting in patients with continuous renal replacement therapy(CRRT). Methods 425 patients with CRRT were involved in the study. We built a predictive risk model of extracorporeal blood clotting with the 302 participants, and 103 participants were used to validate the model. The primary endpoint of CRRT was extracorporeal circulation pipe blockage. Results We used a score of 0-5 point evaluating system to predict the risk of 24 hours CRRT integral model of cardiopulmonary bypass clogging. The area under the CRRT predictive model of cardiopulmonary bypass clogging integral system ROC curve was 0.790 (95% CI 0.719-0.826)(P<0.001). The evaluating system can determine the blockage of 24 hours CRRT extracorporeal circulation. The results showed that CRRT extracorporeal plugging prediction fitted the integral model and could predict the chance of plugging. The actual plugging rate showed no significant difference from the predicted rate (R2=0.301, P=0.232). The cardiopulmonary pipe survival time between the 3 groups(low risk, intermediate risk, and high risk) showed a significant difference (P<0.05). Conclusion We established a continuity extracorporeal blood purification plugging risk score model, to predict plugging risks during CRRT treatment.
