Abstract： With the increasing attention to primary care services and the growing of society needs，a large number of scientific research questions have emerged in the field of general practice and community health. This problem-based scientific research promotes the construction and development of the discipline. The discipline connotation characteristics and professional thinking elements of general practice belonging to the scope of clinical medicine and community health focusing on public health determine the design focus and research methods of scientific research. However，the common problem of scientific research in general practice and community health in China is the lack of attention to the characteristics of scientific general practice research thinking and insufficient depth of thinking. This paper explains the implemention of scientific research in detail guided by general practice thinking in three aspects including the identification and location of research questions，the building of multidisciplinary research team to achieve technology crossover and the development of holistic and systematic research design based on the elements of general practice thinking（"human-centred"，holistic approach，systematization），supplemented by the practice cases of the department of general practice of Yangpu Hospital，Tongji University，in order to provide a reference for researchers on general practice and community health to implement scientific research focus on the connotation characteristics，and contribute to the positive development of scientific research in general practice.

Abstract： Background As a common neurodegenerative disease, Parkinson's disease (PD) has a long course and seriously affects the quality of life of patients.Internationally, a whole-course management mode of PD patients in cooperation between specialists and GPs has been established,which can effectively improve the quality of life of patients and reduce the burden on families and society. China's health management system is different from foreign countries, and there is no PD community management program suitable for China.Objective To explore and construct PD community standard management scheme suitable for GPs in China. Methods Domestic and foreign literatures related to PD diagnosis and treatment management, including guidelines, norms and consensus, were searched. Further through expert interviews, combined with evidence-based basis, formulate PD community standard management program discussion items. Delphi expert letter consultation method was used to construct PD community standard management scheme suitable for GPs in China. Results The positive coefficients of the two rounds of Delphi letter consultation were 100.0%, and the authority degree of the two rounds was 0.84 and 0.85 respectively. The coordination coefficient P values of the two rounds of questionnaires were all less than 0.05, which were significant. Finally, a standardized management scheme for PD community with 77 items was constructed, including "Managed objects and contents", "Suggestions for upward referral", "Suggestions for receiving referral" and "Disease management". Conclusion The two rounds of letter consultation have a high degree of positivity and authority. After the two rounds of letter consultation, the opinions of experts tend to be consistent, the coordination of expert opinions is good, and the results are credible. Finally, the standardized PD community management program can guide GPs to participate in clinical follow-up and daily management of PD, so as to improve the PD management service level in the community and reduce the economic pressure of PD patients.
