  • Consistency Analysis of Imaging and Histological Diagnosis of Non-alcoholic Fatty Liver Disease

    Abstract: Background Nonalcoholic fatty liver disease(NAFLD)is a common digestive system disease,which is often diagnosed by imaging methods in clinical work. At present,there is a lack of research on the effectiveness evaluation of imaging diagnostic methods with histology as the gold standard. Objective To study the consistency of imaging and histological diagnosis of NAFLD,and to analyze the influencing factors of gallstone disease(GD)complicated with NAFLD. Method From January 2021 to July 2022,53 patients with GD who underwent simple cholecystectomy and liver biopsy were selected from the Department of Hepatobiliary surgery,Nanjing Hospital affiliated to Nanjing University of traditional Chinese Medicine. The demographic characteristics,BMI,systolic blood pressure(SBP)and diastolic blood pressure(DBP)of the subjects were collected,and the laboratory indexes,imaging and histological results were collected. The consistency of imaging and histological diagnosis of NAFLD was evaluated by Kappa consistency test. According to the histological diagnostic criteria,the subjects were divided into NAFLD group(n=15)and non-NAFLD group(n=38). Multivariate Logistic regression analysis was used to explore the influencing factors of NAFLD in patients with GD. Results The imaging detection rate of NAFLD in GD patients was 20.7%(11/53),which was lower than that of histology(28.3%)(15/53)(Kappa=0.404,P=0.001). The sensitivity of imaging diagnosis was 60.0%(9/15),the specificity was 94.7%(36/38),and the rate of missed diagnosis was 40.0%(6/15). There were 8 cases of nonalcoholic fatty liver(NAFL),7 cases of nonalcoholic steatohepatitis(NASH) and no NASH associated cirrhosis in 15 patients with histological manifestation of NAFLD. The proportion of fibrosis in the NASH group was higher than that in the NAFL group. There were significant differences in the levels of BMI,SBP,fasting blood glucose(FPG)and aspartate aminotransferase(AST)between NAFLD group and non-NAFLD group(P<0.05). Multivariate Logistic regression analysis showed that BMI(OR=1.5,95%CI=1.084~2.075,P=0.014)and FPG(OR=2.163, 95%CI=1.246~3.756,P=0.006)were the influencing factors of GD patients with NAFLD. Conclusion Compared with histological diagnosis,the missed diagnosis rate of imaging diagnosis of NAFLD is higher,and the incidence of NAFLD may be underestimated by imaging diagnosis as a means of epidemiological investigation. High BMI and high FPG are the main risk factors of GD complicated with NAFLD.

  • Mechanism and in Vitro Experiment of Wogonin in Treatment of Hepatocellular Carcinoma Based on Network Pharmacology

    Abstract: Background Hepatocellular carcinoma(HCC) is the leading cause of cancer-related deaths. The current prevention and treatment situation remains critical. It is of scientific significance to explore new therapeutic agents for HCC. Objective To analyze the mechanism of wogonin on HCC by network pharmacology and to verify it in vitro. Methods Thedrug targets of wogonin were searched in TCMSP database,and the disease targets of HCC were collected from TTD,GenCard, OMIM,DisGent databases. The collected drug targets and disease targets were intersected as potential targets for drug intervention in diseases. R software was used for enrichment analysis of intersection targets,STRING database and Cytoscape software were used to construct protein interaction network and screen core targets. The core targets were further analyzed in GIEPA database. Finally,the preliminary analysis results were verified by in vitro experiments,including cell activity determination using CCK- 8 kit,cell proliferation determination using plate clone formation experiment,cell migration determination using scoring test,protein expression level determination using Western-blotting(WB) assay. Results The AMDE characteristics of wogonin were found to be in accordance with the rules for small molecule drug formation and the toxicity analysis showed no toxicity. A total of 135 wogonin targets and 8238 HCC targets were collected,and 113 targets were intersected. Through the analysis of the core genes of TOP10 screened by the constructed protein interaction network,it was found that the mRNA levels of CDK1 and SRC in liver cancer tissues were higher than those in normal liver tissues(P<0.05),and the high expression levels in liver cancer patients were related to poor prognosis(P<0.05). KEGG enrichment analysis showed that the intersection genes were enriched in the PI3K/AKT signaling pathway,and the molecular docking results showed that wogonin had strong binding configuration activity with CDK1 and SRC. The results of CCK-8 kit showed that the activity of HepG2 cells in the 75,150,and 300.0 μmol/L wogonin groups was lower than that in the control group(P<0.05). The results of plate clone formation experiment showed that the number of colony formation of HepG2 cells in the 37.5,75,150.0 μmol/L wogonin groups was lower than that in the control group(P<0.05). The results of scoring test showed that the migration rate of HepG2 cells in the 37.5,75 and 150.0 μmol/L wogonin groups was lower than that in the control group (P<0.05). The results of the WB assay showed that the expression levels of PI3K,P-AKT/AKT,CDK1 and SRC proteins in the 75 and 150.0 μmol/L wogonin groups were lower than those in the control group(P<0.05). Conclusion Wogonin inhibits the proliferation and migration of HCC cells and induces apoptosis by down#2;regulating the expression of CDK1 and SRC proteins and attenuating the PI3K/AKT pathway signaling,to achieve the purpose of interfering with the occurrence and progression of HCC.

    Abstract: As one of the two wings of innovation and development, science popularization has become one of the most fundamental factors determining the level of material culture development and national creativity. Academician of Chinese Academy of Sciences and Academicians of Chinese Academy of Engineering (Academicians for short), as outstanding role models of China’s scientific and technological community, have actively borne social responsibility for science popularization and played an important role in different development periods. Entering a high-quality development stage, promoting the transformation of science popularization concepts, achieving high-quality science popularization supply, constructing a large-scale science popularization pattern, and planning a large-scale science popularization strategy to address the many challenges brought about by the knowledge explosion and the digital era and promte the process of Chinese modernization, have become an important task. Based on the analysis of the characteristics of academic popularization and the experience of popular science practice among academicians, this article reinterprets the role and significance of academic popularization from a new historical perspective. It also puts forward suggestions for better playing the role of academicians and promoting the prosperity and development of science popularization.
