  • Performance and Retention Intention Rates of Order-oriented Medical Students in China:a Meta-analysis

    Abstract: Background In order to change the shortage of general practitioners in primary care,China has been implementing a rural order-oriented medical student training program in 2010. Currently,the overall performance intention, performance situation and retention intention of order-oriented medical students in China still remains unclear and need to be supported by more reliable evidence. Objective To systematically evaluate the performance intention rate,actual performance rate and retention intention rate of order-oriented medical students in China and analyze its influencing factors. Methods From January to June 2022,CNKI,Wanfang Data,VIP,PubMed and Embase databases were searched to collect literature related to service intention,performance intention and retention intention for primary care of order-oriented medical students in China with a time period from 2010-01-01 to 2021-12-01,and the language of the literature was limited to Chinese and English. Two investigators independently screened literature and extracted data independently to evaluate the risk of bias of the included studies. Meta-analysis of single-group rates was performed using RevMan 5.4 software. Results A total of 49 papers were included with a survey period of 2010-2020,involving 22 413 order-oriented medical students. The performance intention rate of order-oriented medical students was 62%[95%CI(55%-69%)],the performance rate was 95%[95%CI(93%-96%)],the retention intention rate was 16%[95%CI(13%-19%)]. The results of subgroup analysis showed that the performance intention rate of the literature published in 2011[90%,95%CI(84%-96%)] was higher than the other years(P<0.01),the performance intention rate was higher in North China[87%,95%CI(84%-90%)] than the other regions,the performance intention rate of college students[68%,95%CI(67%-69%)] was higher than the residents in standardization training(P<0.01);the performance rate of the published literature in 2020 [99%,95%CI(99%-100%)] was higher than the other years(P<0.01), and the performance rate[100%,95%CI(99%-100%)] in Central China was higher than the other regions(P<0.01); the retention intention rate of the literature published in 2015[36%,95%CI(2%-70%)] was higher than the other years(P<0.01), the retention intention rate[23%,95%CI(4%-41%)] was higher in South China than the other regions(P<0.01),the retention intention rate of the college students[18%,95%CI(13%-22%)] was higher than the residents in standardization training and primary care providers performing contracts(P<0.01). Conclusion The actual performance rate of order-oriented medical students in China is high,but the performance intention rate and retention intention rate are low. Region,time and medical education stage are the influencing factors.

  • A Study of Online Health Community Medication Counseling Based on Text Mining

    Abstract: [Purpose/Significance]Online health community has become an important way for people to obtain health information, and studying the demand of medication consultation of online health community users contributes to the optimization and sustainable development of online health community drug service. [Method/Process]Taking 39Health.com as an example, firstly, 59,048 medication consultation comments of gastrointestinal medicines are crawled using Python coding and pre-processed; secondly, the experimental data are subjected to theme keyword mining using text mining methods such as TF-IDF, TextRank, and LDA topic model, and keyword co-occurrence network analysis is carried out; lastly, a comprehensive analysis of the online health community users’ medication consulting needs of the theme characteristics, and put forward optimization suggestions. [Result/Conclusion] The results of this study show that online health community users are mainly concerned about the therapeutic effect of drugs, the way of taking drugs, adverse reactions, the difference between drugs, and the precautions to be taken during pregnant women and other special groups using drugs. This study provides a theoretical basis for drug manufacturers to adjust and optimize the content of drug manuals, and on the other hand, it provides a direction for online health communities to optimize the content layout of drug manuals and to establish or improve drug popularization services.

  • Integrating Network Pharmacology And Experimental Verification To Explore The Mechanism Of Qionggui Power Against Atherosclerosis

    Abstract:芎归散(QP)是中医药中的经典处方,在过去几十年中已显示出治疗动脉粥样硬化的潜力。然而,介导这些心血管益处的机制仍有待完全阐明。在这里,我们利用网络药理学方法和体外模型研究了QP对动脉粥样硬化的作用和机制。从公共数据库中收集QP的活性成分和相关靶点。通过广泛应用生物信息学方法,包括蛋白质-蛋白质相互作用(PPI)网络、基因本体学(GO)和京都基因和基因组百科全书(KEGG),确定了QP对抗AS的中枢靶点和信号通路。在LPS刺激的小鼠巨噬细胞RAW264.7中验证了预测的主要靶点。在该模型中还评估了QP的抗炎特性。通过对QP的电子调查,确定了18种活性成分和49种与AS相关基因交叉的化学靶点。KEGG途径分析显示这些化学靶点的脂质和动脉粥样硬化途径高度富集。生化分析显示,QP对预测的化学靶点(PPARr、CAT、PTGS2)和LPS诱导的炎症基因(IL1、IL6和TNF)的表达有显著影响。这些抑制作用与LPS刺激激活的NF-B信号通路的抑制有关。我们的发现揭示了QP在预防和治疗动脉粥样硬化方面的治疗潜力。

    Abstract: Human sense of stimulus quantity is divided into quantitative sense and qualitative sense. Whether it's Weber- Fechner's logarithm sensory law or Stevens's power function sensory law are both such a law that is about the quantitative relationship between sensory quantity and one-dimensional luminance stimulus. The image belongs to the stimulus with two-dimensional distribution characteristics of the luminance. It is studied in this paper that is the qualitative sense for two-dimensional luminance stimulus, that is, the sense of the good level or bad level of the quality of two-dimensional luminance stimulus are studied. Good level or bad level is a fuzzy psychological conception, so we need to use the method of fuzzy mathematics to quantify the good level or the bad level of the visual perception quality of an image, that is to establish a fuzzy membership function PQ to quantitatively represent the good level or the bad level of the visual quality of an image.

  • Pathophysiology-based mechanism and management strategies for deadly leaking lungs caused by 2019 novel coronavirus

    Abstract: The 2019 novel coronavirus has killed 2596 globally since the outbreak began in December 2019 and more than 11,000 patients are still in severe conditions. The virus and medical conditions caused by the virus were named as SARS-CoV-2 and COVID-19. Although antiviral, symptomatic, and functionally supportive treatments have been applied, more than 100 patients die each day from infection with the virus. The most common deadly complication of COVID-19 is acute respiratory distress syndrome (ARDS). SARS-CoV-2 infection may become pandemic. ARDS is caused by various similar viruses, such as severe acute respiratory syndrome (SARS), Middle East respiratory syndrome (MERS), and influenza viruses. Thus, strategies for decreasing the mortality of virus-initiated ARDS are needed at the urgent, long-term, and broad levels. ARDS was previously referred to as wet lungs with vascular leaking, which described the clinical manifestations and chest computed tomography images of COVID-19-ARDS. Determining from where and how water is leaking into the lungs will lead to the development of mechanism-based management strategies for reducing mortality. These approaches can be used to prevent and stop water leakage from the pulmonary capillary into the lung interstitial space. For mild and moderate cases, nonsteroidal anti-inflammatory drugs, such as those used for rheumatoid arthritis, may be useful for preventing and deceasing this water leakage. Administration of immunosuppressants, such as sirolimus and tacrolimus, may stagger the peak times of injuries caused by viral infection, immune response, and non-immune inflammation to decrease the extent of water leakage and prevent life-threatening conditions. Using anti-flame (Qing-Huo) Chinese herbs may also be useful. For severe case, blood dialysis can be an effective strategy for eliminating most inflammatory mediators and cytotoxic substances.

  • T-Area-Marker for Scientific Images

    Abstract: Labeled images are one of the most important means of scientific communication and education. However, traditional markers (arrows, lines) are point markers; do not include information about how large the feature is. We designed an efficient marker system for labeling scientific images (electron or light microscopy, CT, MRI, ultrasonography, camera pictures, etc), called the “T Area Marker, (TAM)”. The basic TAM marker looks like a “T”, composed of a line segment and a small tick on one end; it defines an imagined circle that stands on the tickless end and the diameter of the circle is equal to the length of the line segment. Thus the TAM can define an exact area rather than a single point; and the imagined circle does not break the continuity of the image (unlike traditional visible circles, rectangles, etc). A TAM with N ticks (N>1) means the diameter equals to N times the length of TAM. A TAM may also have a tail and/or several tail branches to define translation of the imagined circle, thus define complicated areas. tAreaMarker.py is free software that combines the drawing and reading of TAMs, although in most cases TAMs are easily interpreted without computer assistance.
