  • Construction of case management model combined with proprioceptive training in the rehabilitation of patients with knee arthroplasty

    Abstract:

    Objective To explore the application effect of case management mode construction combined with proprioceptive training in the rehabilitation of patients with knee arthroplasty.

    Methods 76 patients who underwent unilateral knee arthroplasty for the first time in the Department of orthopedics of Hai'an Hospital of traditional Chinese medicine from July 2019 to December 2021 were selected as the research objects. The patients were divided into control group and observation group according to the odd and even number at the end of the medical record number. The control group adopted the routine nursing mode, and the observation group added the case management mode combined with proprioceptive training for perioperative management on the basis of the control group. The first time out of bed, hospital stay and hospitalization expenses of the two groups were observed. The static and active visual analogue scale (VAS) scores of the two groups before and after treatment were compared, and the HSS scores of the two groups before, 1 week, 3 weeks and 1 month after operation were compared.

    Results The first time out of bed, hospital stay and hospitalization expenses in the observation group were lower than those in the control group; The VAS scores of static and activity in the observation group were lower than those in the control group; The HSS scores of patients in the observation group before operation, 1 week and 3 weeks after operation were higher than those in the control group; The difference was statistically significant (P < 0.05).

    Conclusion The construction of case management mode combined with proprioceptive training can effectively promote the postoperative rehabilitation of patients with knee arthroplasty, relieve pain, enhance knee function and improve the walking ability of patients. At the same time, the length of hospital stay can be shortened and the hospitalization cost can be reduced.

  • A Novel Criterion of the Transformer Differential Protection based on the Hausdorff Distance Algorithm

    Abstract: The characteristics of whole waveforms of magnetizing inrush current and fault differential current were analyzed. By virtue of the similarity difference between the two currents and combining the superiorities of the Hausdorff distance algorithm, a novel criterion of the transformer differential protection based on the Hausdorff distance algorithm was proposed. The Hausdorff distance between the normalized differential current sequence and standard sinusoidal wave sequence was applied to distinguish between inrush current(including symmetrical inrush current) and fault differential current(including fault differential current superimposed with inrush current). The effectiveness, rapidity and anti-interference performance of the proposed criterion were validated with simulation tests.

    Abstract: Herba Xanthii is a traditional folk medicinal plant.The 50% ethanol eluting faction of Xanthium mongolicum in Jiangxi had significant activity to inhibit necroptosis. In order to investigate the chemical constituents, the active fractions of X. sibiricum were isolated and purified by various chromatographic methods such as repeated silica gel column chromatography, Sephadex LH-20 column chromatography, preparative high performance liquid chromatography and recrystallization to yield 14 compounds. Their structures were established on the basis of NMR, MS spectraoscopic analyses and comparison with lirerature date. Fourteen compounds were isolated from the active part and identified as hydroxydihydrobovolide(1), raspberry ketone(2), salicyl alcohol(3), 4-hydroxyl-acetophenone(4), 4-hydroxybenzaldehyde(5), ethyl caffeate(6), ferulaldehyde(7), isoscopletin(8), 3,3'-bis(3,4-dihydro-4-hydroxy-6 -methoxy-2H-1-benzopyran)(9),axillarin (10),quercetin (11),(+) pinoresinol(12), β-sitosterol(13) and palmatic acid(14). Compounds 1, 2, 3, 4, 7, 8, 9, 10 were isolated from Xanthium mongolicum for the first time.

    Abstract: Cinnamomi Ramulus is a traditional chinese medicine commonly used in clinic. The ethanol extracts of Cinnamomi Ramulus had found the physiological activity to inhibit necroptosis in the previous studies. In order to clarify the chemical constituents of the plant and provide scientific basis for the rational development and sustainable utillization of the plant resources, the75% ethanol extracts of Cinnamomi Ramulus were isolated and purified by various chromatographic methods such as macroporous adsorption resin, silica gel, Sephadex LH-20 and preparative HPLC to yield 13 compounds. Their structures were identified by physicochemical properties and various spectral data analyses as abscisic acid(1), grasshopper ketone (2), 2,3-dihydroxy-1-(4-hydroxy-3,5- dimethoxyphenyl)-1-propanone (3), erythro-type-1,2,3- trihydr- oxyphenylpropane (4), 1-phenyl-1,3-propanediol (5), coumarin (6), cinnamic acid (7), p-hydroxycinnamic acid (8), o-hydroxycinnamic acid (9), o-metho- xycinnamic acid (10), cinnamaldehyde (11), ferulic acid (12) , ethyl caffeic acid (13). Among them, 1-5, 12 and 13 were isolated from the plant for the first time.

    Abstract:毛梗豨莶草(Siegesbeckia glabresce)的乙酸乙酯萃取部位具有显著的抑制细胞坏死性凋亡(necroptosis)的活性。为明确毛梗豨莶草的活性成分,对毛梗豨莶草乙酸乙酯萃取部位进行了化学成分分离。采用正相硅胶柱色谱谱( 二氯甲烷∶甲醇20∶1~0∶1)、反相ODS柱色谱(30%~100% 甲醇),Sephadex LH-20柱色谱、重结晶等方法进行分离和纯化,根据理化性质和波谱数据进行结构鉴定。从毛梗豨莶乙酸乙酯部位分离并鉴定了9个化合物,分别为3,7-二甲氧基槲皮素(1),芹菜素(2),奥卡宁(3),okanin-4'-O-β-D-6''-trans-p-coumaroylglucoside(4),1H-Indole-3-carbaldehyde(5),对羟基苯甲醛(6),3,5-二甲氧基-4-羟基苯甲醛(7),3,4-divanillyltetrahydrofuran(8),buddlenol D(9)。除化合物1和6外,其余7个化合物均为首次从豨莶属植物中分离得到。
