  • Efficacy and Safety of Dual-targeted Chimeric Antigen Receptor-T Cell Therapy in Patients with Refractory-relapsed Multiple Myeloma:a Meta-analysis

    Abstract: Background Chimeric antigen receptor(CAR)-T cell immunotherapy has achieved good therapeutic effect in multiple myeloma(MM),and the most common target is B cell maturation antigen(BCMA). The disadvantage of single target CAR-T cell immunotherapy is that it can lead to disease resistance and recurrence,which may be related to antigen escape. Therefore,the dual-targeted CAR-T cell therapy for refractory-relapsed multiple myeloma(RRMM)has been improved and developed,but there is still a lack of systematic clinical analysis in this field. Objective A meta-analysis was conducted on the efficacy and safety of dual-targeted CAR-T cell therapy for RRMM patients. Methods PubMed,Embase,Cochrane Library,Web of Science,CNKI,Wanfang,and VIP were searched for single-group rate studies on dual-targeted CAR-T cell therapy in patients with RRMM from inception to 2023-02-06. The Data were extracted for collection by 2 investigators using a self-designed form and the quality of literature was evaluated using the methodological index for non-randomized studies (MINORS). The data analysis was conducted using RStudio software. Results A total of 9 clinical studies,involving 200 RRMM patients who had previously received multi-line therapy were included in the study. Dual-targeted CAR-T cell therapy can be mainly divided into four categories based on different targets of BCMA/CD19,BCMA/CD38,BCMA/TACI,and BCMA/CS1, of which the BCMA+CD19 target is more studied. Dual-targeted CAR-T cell therapy also can be divided into four categories of bispecific categories,combined/sequential infusion of two different CAR-T cells,bicistronic or cotransduction according to the different forms of infusion. Meta-analysis showed that the overall response rate(ORR)of dual-targeted CAR-T cells for RRMM was 90%(95%CI=0.849-0.943),and the complete response rate(CRR)was 54.6%(95%CI=0.416-0.673),the negative rate of minimal residual disease(MRD) was 75.6%(95%CI=0.489-0.952),the ORR of extramedullary diseases(EMD) was 55.1%(95%CI=0.234-0.851),the recurrence rate at the last follow-up was 29.7%(95%CI=0.141-0.454),and the survival rate was 75.6%(95%CI=0.554-0.915). The incidence of grade 3 to 4 cytokine release syndrome(CRS)was 16.4% (95%CI=0.094-0.245),and the incidence of immune effector cell-associated neurotoxicity syndrome(ICANS) was 4% (95%CI=0.094-0.245). Sensitivity analysis suggested stable results. The results of Egger's test indicated a potential bias risk for ORR(P=0.03) and overall response rate of EMD(P=0.02). Meanwhile,no publication bias was suggested for CRR(P=0.53),MRD negative rate(P=0.79),recurrence rate at the last follow-up(P=0.71),survival rate(P=0.98),incidence of grade 3-4 CRS(P=0.90),and incidence of ICANS(P=0.30). Conclusion Dual-targeted CAR-T cell therapy for RRMM has shown favorable efficacy and safety,and multicenter,large-sample,and longer follow-up studies are needed to further evaluate its efficacy and safety.

  • Survival and Prognosis Analysis of Bortezomib Based Regimen in Newly Diagnosed Super-Aged Multiple Myeloma Patients

    Abstract: Background  Multiple myeloma is a highly heterogeneous disease. Super-aged patients with multiple myeloma are a special group. There is a lack of evidence-based medical basis in the treatment decision and general evaluation of them,and there is also some controversy in the assessment of frailty,so there is need for further research. Objective  To explore the clinical features and the influencing factors of survival and prognosis in newly diagnosed super-aged multiple myeloma(MM) patients with the bortezomib based regimen and to evaluate the best assessment model for the general status of patients with MM. Methods  The clinical data of newly diagnosed super-aged patients with MM in Beijing Chao-Yang Hospital from November 2013 to January 2023 were retrospectively analyzed. Survival follow-up was conducted through the medical record system of Beijing Chao-Yang Hospital until April 1,2023,with the end points of this study of overall survival(OS)and progression-free survival(PFS). The patients were divided into the two-drug treatment group(n=18)and three-drug treatment group(n=11),and the clinical and genetic characteristics of the two groups were compared. The Geriatric Assessment System(GA)score, UK Myeloma Research Alliance Risk Profie(MRP)score,and Mayo score were used to evaluate the frailty status and treatment efficacy. Survival curves of OS and PFS in MM patients were plotted using the Kaplan-Meier method. Log-rank test was used for univariate analysis of the influencing factors of OS and PFS. Multivariate Cox hazard risk regression model was used to analyze the influencing factors of OS and PFS in MM patients. Results  The median PFS was 8.70 months(1.90-43.87 months)and the median OS was 17.23 months(2.00-72.83 months). A total of 21(72.41%)patients experienced disease progression(PD)or relapse,and 12(41.38%)patients died by the final follow-up. The objective remission rate(ORR)for first-line treatment was 82.76%(24/29),the partial remission(PR)rate was 51.72% (15/29),the very good partial remission(VGPR)rate was 24.14%(7/29),and the complete remission (CR)rate was 6.90%(2/29). There was no significant difference in CR rate,VGPR rate,PR rate and ORR between the two groups of newly diagnosed super-aged MM patients(P>0.05). Multivariate Cox hazard risk regression analysis showed that MRP debilitation(HR=0.213,95%CI=0.049-0.924,P=0.039),elevated serum corrected calcium(HR=0.153,95%CI=0.041-0.570,P=0.005)and maintenance therapy(HR=4.301,95%CI=1.219-15.169,P=0.023)were independent influencing factors of PFS in newly diagnosed super-aged MM patients,while maintenance treatment(HR=4.372,95%CI=1.049-18.221,P=0.043)was an independent influencing factor for OS in newly diagnosed super-aged MM patients. Conclusion  There is no significant difference in the efficacy and prognostic impact of two-drug or three-drug Bortezomib based treatment. Serum corrected calcium elevation and maintenance therapy are independent prognostic factors for survival. MRP score can be used to assess the prognosis of newly diagnosed super-aged MM patients.

  • Relationship between Homocysteine Level and Acute Kidney Injury in Patients with Acute Ischemic Stroke

    Abstract: Background Acute ischemic stroke (AIS) is the second leading cause of death worldwide after coronary heart disease. Acute kidney injury (AKI) is one serious complication after AIS,and homocysteine (Hcy) may be an important factor associated with AKI and accelerated deterioration of renal function. However,there are few studies on the relationship between Hcy and AKI,especially in patients with AIS. Objective To investigate the relationship between plasma Hcy level and AKI in patients with AIS,and to provide new ideas for the prevention and treatment of AKI. Methods Baseline clinical data of 1 202 patients with AIS who were admitted to Department of Neurology,the Second Hospital of Tianjin Medical University were collected from the electronic medical record systemfrom January 2018 to April 2021. Patients were divided into normal Hcy(Hcy ≤ 15 μmol/L, n=618),mild hyperhomocysteinemia (HHcy)(15 μmol/L30 μmol/L, n=125)groups according to the Expert Consensus on the Diagnosis,Treatment,and Prevention of Hyperhomocysteinemia. Patients were divided into AKI group and non-AKI group by the values of ambulatorily monitored renal function and urine volume within seven days after admission recommended in the KDIGO 2021 Clinical Practice Guideline for the Management of Glomerular Diseases. Multivariate Logistic regression was used to explore the effects of Hcy on post-AIS AKI as a continuous variable and a categorical variable,respectively. Subgroup analysis was used to investigate the relationship between Hcy and AKI in subgroups. The nonlinear relation between Hcy and AKI was explored by restricted cubic spline regression. Results One hundred and fifty patients (12.48%) developed AKI in all subjects. Multivariate Logistic regression showed that after adjustment for potential confounders, the risk of AKI increased by 1.035 times〔 OR=1.035,95%CI(1.019,1.052), P<0.05〕 for every 1 μmol/L increase in Hcy. With reference to normal Hcy,mild and moderate-to-severe HHcy has been associated with a 1.770-fold〔 OR=1.770,95%CI(1.150,2.724), P<0.05〕and 2.927-fold 〔 OR=2.927,95%CI(1.671,5.126), P<0.05〕 increased risk of AKI,separately. Subgroup analysis found that the risk of AKI after AIS increased with the increase of Hcy level (used as a continuous variable)in females,those aged ≥ 75 years,those with hypertension,diabetes or moderate to severe stroke at admission,and those whose stroke type was large-artery atherosclerosis (LAA),small artery occlusion (SAO) or cardio embolism (CE)( P<0.05). When Hcy was analyzed as a categorical variable,mild HHcy was associated with a higher risk of AKI compared with normal Hcy in the male population,those aged<75 years,those with hypertension,diabetes,a history of stroke or mild stroke at admission,and those without coronary heart disease( P<0.05). And moderate-to-severe HHcy was associated with a higher risk of AKI compared with normal Hcy in the female population,those with hypertension,diabetes,or moderate or moderate-to-severe stroke at admission,and those whose stroke type was LAA,SAO or CE regardless of age,coronary heart disease and history of stroke( P<0.05). Restricted cubic regression manifested that there was a nonlinear correlation between Hcy and the risk of AKI,and the curve was convex( P=0.026). The risk of AKI after AIS increased rapidly with the increase of Hcy when admission Hcy was less than 17 mmol/L,but increased slowly with the increase of Hcy when admission Hcy was greater than or equal to 17 mmol/L.Conclusion Elevated Hcy is a risk factor for AKI whether as a continuous variable or a categorical variable in AIS patients. So monitoring the level of Hcy is conducive to early identification and prevention of AKI,which is helpful to improve the prognosis in AIS patients.

  • Urban Sustainable Development Evaluation with Big Earth Data: Data, Indicators, and Methods

    Abstract: The sustainable development of cities is the key to achieving the United Nations 2030 Sustainable Development Goals (SDGs). There have been numbers of studies on urban sustainable development assessment, but the existing indicators are usually based on traditional statistical data, which makes urban sustainable development assessment limited in global city assessment and comparison. Big Earth Data can overcome the problems of traditional data statistics such as inconsistent statistical caliber, incomplete statistical information, and difficulty in obtaining data. With its macroscopic, dynamic, and diverse advantages, it can provide new assistance for urban sustainable development assessment. Based on the research on sustainable development of cities worldwide, this study sorts out the SDG 11 and other goals related to urban sustainability, and discusses the possibility of Big Earth Data, such as remote sensing data and the Internet big data, in the urban sustainable development evaluation. This study hopes to provide a scientific reference for the sustainable development of cities. The sustainability indicators using the Big Earth Data has realized the integration and utilization of multi-source information, which will help to achieve more quantitative, real-time, and detailed evaluation for urban sustainability.

  • Integration of Remote Sensing Observations and Models Supports the Regional Sustainable Development in Heihe River Basin, China

    Abstract: The Heihe River Basin is the second largest inland river basin in China. During the last few decades, this region has been faced with a serious of severe ecological and environmental problems caused by rapid economic growth and climate change. Two decades ago, with the increase of population and water consumption in the upper and middle reaches, the ecological water consumption in the lower reaches was greatly squeezed, which eventually led to serious ecological disasters, such as the shrinkage of the natural oasis, the drying up of the lakes, and frequent sandstorms in the lower reaches. The sustainable development of the region has been seriously challenged. To accurately monitor changes in hydrological and meteorological variables at watershed scale and support the wise use of water resources in this region, the Heihe Remote Sensing Experimental Research Station (HRSERS) was founded in 2009. The foundation of HRSERS has greatly improved the level of scientific research and decision support in the Heihe River Basin. The main achievements include: (1) the first watershed scale (with an area of 143000 km2) integrated observation system in China is constructed, and a large number of long-term observation data are obtained. More than 200 papers have been published using the observed dataset; (2) key eco-environmental variables in Heihe River Basin are retrieved from remote sensing data; (3) the theoretical framework and method system for the authenticity inspection of heterogeneous surface remote sensing products have been established, and some related national standards have been approved; (4) the eco-hydrological model is improved by adding glacier, snow and permafrost modular, and the eco-hydrology-economic coupling model is developed for the whole Heihe River Basin to full couple the natural processes and social processes; (5) the United Nations Sustainable Development Goals (SDGs) were monitored at the basin scale and regional scale, and provides scientific support for the sustainable development and decision-making of Heihe River Basin.

  • Developing Cryospheric Remote Sensing, Promoting Scientific Programme of Earth’s Three Poles

    Abstract: Remote sensing of cryosphere, as an important branch of cryospheric science, is a new interdiscipline, involving the knowledge of both cryospheric science and remote sensing techniques. This paper overviews the objects and basic methods of cryospheric remote sensing, introduces the progresses by using representative case studies, and especially, describes the research status of Chinese cryospheric remote sensing. Finally, we propose several recommendations for the development of remote sensing of cryosphere. In particular, we discuss the potential paths to the scientific programme of Earth’s three poles.

  • The relationship between homocysteine level and acute kidney injury in patients with acute ischemic stroke

    Abstract:

    Background Acute ischemic stroke (AIS) is the second leading cause of death worldwide after coronary heart disease. Acute kidney injury (AKI) is one of the serious complications, and homocysteine (Hcy) may be an important factor leading to AKI and accelerating deterioration of renal function. However, there are few studies on the relationship between Hcy and AKI, especially in patients with AIS. Objective To investigate the relationship between plasma homocysteine (Hcy) level and acute kidney injury (AKI) in patients with acute ischemic stroke (AIS), and to provide new ideas for the prevention and treatment of AKI. Methods 1202 patients with AIS admitted to the Department of Neurology, the Second Hospital of Tianjin Medical University were selected from January 2018 to April 2021. Patients were divided into normal Hcy, mild hyperhomocysteinemia (HHcy) and moderate-severe HHcy group according to plasma Hcy levels. Multi-factor logistic regression was used to explore the effects of Hcy on post-AIS AKI as a continuous variable and a classified variable respectively. Subgroup analysis was used to investigate the relationship between Hcy and AKI in subgroups. The nonlinear relation between Hcy and AKI was explored by restricted cubic spline regression. Results 150 patients (12.48%) developed AKI in all subjects. Multi-factor logistic regression showed that after adjustment for potential confounding variables, the risk of AKI increased by 1.035 times (95%CI: 1.019-1.052) for every 1μmol/L increase in Hcy. With reference to normal Hcy patients, mild and moderate-severe HHcy has been associated with a 1.770-fold (95%CI: 1.150~2.724) and 2.927-fold (95%CI: 1.671-5.126) increased risk of AKI, separately. Subgroup analysis demonstrated that no matter as a categorical variable or a continuous variable, Hcy had a more significant effect on AKI in patients with a history of hypertension, diabetes or stroke than those without underlying diseases. Restricted cubic regression manifested that there was a nonlinear correlation between Hcy and the risk of AKI, and the curve was convex. Conclusions Hcy is a risk factor for AKI whether as a continuous variable or a categorical variable in AIS patients. Monitoring the level of Hcy is conducive to early identification and prevention of AKI, and improve the prognosis in AIS patients.

  • 成像激光雷达标定系统中三维升降转台的性能研究

    Abstract:三维升降转台是高海拔宇宙线观测站(LHAASO)广角切伦科夫望远镜阵列(WFCFA)成像激光雷达标定系统的核心组成部分,主要由慢控子系统、机械子系统和标准零点子系统构成,用于精确控制激光束的出射方向。本文采用高精度数字图像处理技术对三维升降转台的性能进行研究,结果表明其旋转和升降定位精度分别为0.005°和0.056 mm、重复精度分别为0.003°和0.075 mm,满足成像激光雷达标定系统的实验要求。三维升降转台的自动控制巡航功能,有效减少了人工干预,提高了系统远程使用的可靠性和效率。标准零点的校准功能可以有效地监测三维升降转台的定位精度并且在失准的情况下,能够实现远程校准,以确保三维升降转台长期高精度稳定运行。
