Abstract: Background  Knee osteoarthritis is a common clinical condition with a decades-long course. Long term and personalised health management in community hospitals is the best way to prevent and treat KOA,and non-surgical treatment is an effective way to slow joint degeneration and postpone joint replacement surgery. Objective  To understand the population characteristics, clinical staging and treatment modality characteristics of the population attending community hospitals for knee osteoarthritis,in order to provide a basis for optimising the treatment protocol for knee osteoarthritis at the grassroots level. Method  All streets in the urban area of Beijing were randomly selected for the study using the whole cluster sampling method, and Hepingli Street in Dongcheng District,Beijing was censored for all patients who attended all community hospitals(7) belonging to the street in January-June 2022,and general information, medical history, personal history, clinical staging,K-L grading and treatment modalities were collected. Results  A total of 3 615 KOA patients were included in this study, including 1 327 males(36.71%) and 2 288 females(63.29%),aged(71.8±13.3) years. There were 867 cases(23.98%) in the onset period,209 cases(55.57%) in the remission period and 739 cases(20.45%) in the rehabilitation period. Gender and age of patients with different clinical stages were not correlated with clinical stages (P>0.05),while BMI(K=0.235) and KL grading(K=0.406) were correlated with clinical stages(P<0.001). During the attack period,4~5 kinds of treatment methods(48.67%) were combined, and the treatment methods were mainly traditional Chinese patent medicines and simple preparations for external use(68.97%),475 cases(54.79%) of traditional Chinese patent medicines and simple preparations for oral use,and 396 cases(45. 67%) of health education; in the remission stage,2-3 treatment methods were used in combination(48.58%),mainly including 1 084 cases(53.96%) of traditional Chinese patent medicines and simple preparations for external use,1047 cases(52.12%) of health education and 790 cases(39.32%) of cupping; in the rehabilitation stage,2-3 kinds of treatment methods were used in combination(47.23%). The main treatment methods were health education in 488 cases(66.04%),traditional exercises in 286 cases(38.70%),and external use of traditional Chinese patent medicines and simple preparations in 279 cases(37.75%). There was a correlation between clinical stage and type of combination therapy(K=0.356,P<0.05). Conclusion  Patients in community hospitals are mainly in remission, and treatment is often combined with various methods of traditional Chinese patent medicine and simple preparations, and traditional exercises are also widely used.

  • Application of Murtagh Safe Diagnostic Strategy combined with Mind Mapping for Establishing Clinical Thinking in General Practice Teaching Clinic

Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2024-04-15 Cooperative journals: 《中国全科医学》

Abstract: General practice teaching clinic is an important training method to develop clinical thinking and improve general practice diagnosis and treatment ability of general practice residents. In this paper,we used a patient with fatigue as the main manifestation as a teaching case to introduce the Murtagh safe diagnostic strategy proposed by John Murtagh,a famous Australian general medicine expert,to inspire the diagnosis and differential diagnosis of fatigue for the general practice residents in the general practice teaching clinic:(1)What are the common diseases that cause fatigue? (2)What are the important diseases that should not be ignored? (3)What are the easily missed diseases of fatigue? (4)Are there underlying easily masked diseases? (5)Is there something the patient is not saying? Combined with the history, physical examination and laboratory findings,a preliminary diagnosis of antineutrophil cytoplasmic antibody-associated vasculitis causing rapidly progressive glomerulonephritis is highly probable. The timely referral got nephropathology which confirms the etiology of the fatigue was antineutrophil cytoplasmic antibody-associated glomerulonephritis, and achieved satisfactory outcome. Based on the Murtagh safe diagnostic strategy, the instructor helped the general practice residents to construct a systematic knowledge framework for the identification and analysis of fatigue,so as to improve their clinical logical thinking ability and the ability to solve practical clinical problems. Using the mind mapping as an auxiliary tool, the diagnosis and differential diagnosis of the Murtagh safe diagnostic strategy were concretized and visualized,so as to optimize the teaching effect.

  • Adiponectin inhibits AKT signaling and antagonizes arsenic-induced lipid deposition in hepatocytes

Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2024-04-01

Abstract: Objective To investigate the effects and molecular mechanism of recombinant human adiponectin (APN) on arsenic induced lipid deposition in hepatic HepG-2 cells. Methods The experiments were divided into control group, sodium arsenite (SA) exposure group, rAPN intervention + SA exposure group, and protein kinase B (AKT) inhibitor (GSK690693) intervention + SA exposure group. After cultured HepG-2 cells in vitro were treated respectively, the characteristics of cellular lipid deposition were observed by oil red O staining of cells smears, and the methods of biochemical or enzyme-linked immunosorbent assay (ELISA) were applied to determine the levels of intracellular mitochondrial membrane potential (ΔΨm), mitochondrial carnitine palmitoyltransferase-1 (CPT-1) activity, free fatty acids (FFAs) and APN. Western-blotting (WB) was applied to detect the levels of phosphorylated AKT (p-AKT), mitochondrial Glutathione S-transferase K1 (GSTK1) and inflammatory factor Caspase-1. Results Compared with control group, SA exposed group showed intracellular lipid deposition and the decreased levels of mitochondria ΔΨm, CPT1 activity, GSTK1 or APN, while the levels of FFAs, p-AKT and Caspase-1 increased. Compared with SA exposed group, recombinant human APN (rAPN) intervention led to the alleviated lipid deposition in hepatocytes and the increased levels of ΔΨm, CPT1 activity, GSTK1 or APN, while the levels of FFAs, p-AKT and Caspase-1 decreased. Similarly, the intervention of AKT inhibitor showed also the reduced cellular lipid deposition and the upregulated levels of ΔΨm, CPT1 or GSTK1, while the levels of p-AKT and caspase-1 downregulated. Conclusion Arsenic induced lipid deposition in HepG-2 cells is associated with AKT signaling, and APN can antagonize arsenic-induced lipid metabolism disorder and lipid deposition in hepatocytes via inhibiting AKT signaling.

Subjects: Psychology >> Cognitive Psychology Subjects: Physics >> Interdisciplinary Physics and Related Areas of Science and Technology Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2024-01-21

Abstract: The non-exclusive dual-process working model proposed in De Neys’ article effectively makes up for the shortcomings of the existing theories and models, but the explanation of the switching problem and the exclusivity problem in this article cannot constitute a complete rejection of the existing switching mechanism and the traditional fast-slow dual-process model. We believe that a more powerful and logical way of explaining the theory is as follows: human reasoning process is related to the complexity of the problem being faced, and it is also closely related to the reading process of the problem; in the process of reading the problem, the brain will automatically choose different ways of reasoning for the problem of different levels of complexity; and in the process of selection, the principle of minimum energy should still be valid.

This study will systematic review the fundamental properties of copper, related diseases caused by copper overload or deficiency, the association between cuproptosis and cancer, and the treatment of diseases related to copper disorders.

  • Mechanisms related to ferroptosis and its application in cancer treatment

Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2023-10-11

Abstract: Ferroptosis is a type of regulated cell death (RCD) dependent on iron and reactive oxygen species, which is mainly characterized by the accumulation of intracellular lipid peroxides and is different from apoptosis, necrosis and autophagy. It has been found that iron death is closely related to tumor cells, and the therapeutic effect of therapeutic drugs on tumors can be improved by inducing iron death in tumor cells. In this paper, we summarize the mechanism of iron death and the latest progress of iron death and tumor-related studies, with the aim of providing reference for further understanding of the mechanism between iron death and tumors, and providing new ideas and targets for the treatment of related diseases.

  • Downregulation of THBS2 suppresses tumorigenicity and liver metastasis of gastric cancer and increases its sensitivity to trastuzumab

Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2023-09-28

Abstract: Objective: Liver metastasis is one of the major causes of cancer-related death in gastric cancer (GC) patients. This study was to investigate the roles of THBS2 in the tumorigenesis and liver metastasis of GC and the sensitivity of GC to trastuzumab.
Methods: Sequencing was employed to identify the differentially expressed genes in the GC. The THBS2 expression was detected in GC tissues and GC cell lines, its relationships with clinicopathological characteristics were further assessed, and its roles in the malignant behaviors of GC were further investigated in vitro and in vivo, by up-regulating or down-regulating THBS2 expression. The PTEN and its downstream AKT and FAK signaling pathways were investigated in the GC aiming to explore the potential mechanism underlying the regulatory effects of THBS2.
Results: THBS2 expression increased significantly in the GC as compared to the normal gastric tissues, which was related to the distal metastasis and poor prognosis of GC patients. THBS2 expression also increased in the primary GC cells and human gastric cell line NCI-N87. THBS2 downregulation induced mesenchymal-epithelial transition (MET) and PTEN nuclear translocation, which inhibited the metastasis of GC cells. Carmofur promoted PTEN nuclear translocation, inhibiting the metastases and THBS2 downregulation together with carmofur improved the sensitivity of GC to trastuzumab.
Conclusions: THBS2 is overexpressed in GC tissues of patients with synchronous liver metastases and its overexpression is associated with poor prognosis. THBS2 downregulation inhibits the metastases of GC. THBS2 down-regulation and carmofur can be used as a new treatment for the advanced GC with metastases.

Abstract: Objective To study the expression of SMARCA4, BRAF, p53, programmed death receptor 1 (PD-1) and its ligand 1 (PD-L1), and BRAF and neurotrophin tyrosine kinase receptor (NTRK) genes changes in colorectal cancer in Tibet. Methods A total of 64 patients with colorectal cancer resected in Autonomous Region People's Hospital from January 2015 to July 2021 was enrolled in this study. The expression of SMARCA4, BRAF, p53, PD-1, PD-L1 was detected by immunohistochemical(IHC) staining, and detection of NTRK1, NTRK2, NTRK3 fusion gene by fluorescence in situ hybridization (FISH), BRAF V600E gene mutation by polymerase chain reaction (PCR). Results 64 cases of colorectal cancer, with a male to female ratio of 1.21:1 and the mean age of (56.59 ± 13.27) years. 46 cases (46/64, 71.88%) were located in the colon, and 18 cases (18/64, 28.12%) were in the rectum. 60 cases (60/64, 93.75%) were adenocarcinoma, and 4 cases (4/64, 6.25%) were other types. 11 cases (11/64, 17.19%) were in T1 or T2 phase, and 53 cases (53/64, 82.81%) were in T3 or T4 phase; 24 cases (24/64, 37.50%) were found lymph node metastasis. The IHC staining showed that SMARCA41 was partial expression reduced or missing in a case (1/64, 1.56%), BRAF were positive in 4 cases (4/64,6.25%), p53 showed mutant expression pattern in 35 cases (35/64,54.69%). PD-1 expression <10% of tumour background immune cells in 45 cases (45/64,70.31%) , and ≥ 10% in 19 cases(19/64,29.69%). PD- L1 proportion Score (CPS)<10 in 52 cases((52/64,81.25%), and ≥ 10 in 12 cases(12/54,18.75%).None case was detected the fusion genes of NTRK1, NTRK1, and NTRK1, and four cases were detected BRAF V600E gene mutations. No SMARCA4 gene changes was detected in one case with SMARCA4 partial expression missing. The difference between the PD-L1 expression proportion score and the status of mismatch repair /microsatellite instability (MMR/ MSI), as well as the expression level of PD-1 was statistically significant (P=0.001). Conclusions SMARCA4 expression reduced or missing and NTRK fusion gene are rare in colorectal cancer in Tibet. A few cases have BRAF V600E gene mutations, and Pan-TRK and BRAF expression can be used as initial screening methods for NTRK fusion gene and BRAF gene mutation. Cases with mismatch repair deficiency / microsatellite instability-high (dMMR/MSI-H) are more prone to high expression of PD-L1 protein, and the patients with dMMR/MSI-H may benefit from immunotherapy. The high expression of PD-1 is positively correlated with the high expression of PD-L1.

Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2023-08-17

Abstract: Objective To analyze the disease burden and changes in risk factors of common malignant tumors in the female reproductive system in China from 1990 to 2019. Method Using the 2019 global disease burden database data, describe the changes in indicators such as incidence, death, and disability adjusted life years (DALY), early death lost life years (YLL), and disability lost life years (YLD) of cervical cancer, uterine cancer, and ovarian cancer in China from 1990 to 2019. Estimate the levels of different risk factors using the population attributable risk percentage. Results From 1990 to 2019, the incidence rate and mortality of cervical cancer, uterine cancer and ovarian cancer in China showed an upward trend. The incidence rate of ovarian cancer increased by 0.78%, the highest increase. In 2019, the age of high incidence of cervical cancer and uterine cancer was 55 to 59 years old, while the age of high incidence of ovarian cancer was 70 to 74 years old. The DALY, YLL, and YLD of cervical cancer, uterine cancer, and ovarian cancer increased to varying degrees in different age groups. Among the risk factors leading to DALY and DALY rates, cervical cancer is characterized by smoking (145300 person years/20.83%) and risky sexual behavior (1622200 person years/23.257%), uterine cancer is characterized by high body mass index (BMI) (90800 person years/13.02%), ovarian cancer is characterized by high BMI (16700 person years/2.39%), high blood sugar (44600 person years/6.40%), and occupational exposure to asbestos (7900 person years/1.13%). Compared to 1990, the largest increase in China's DALY rate in 2019 was attributed to ovarian cancer, which was attributed to high BMI, high blood sugar, and occupational exposure to asbestos, with increases of 482.93%, 253.59%, and 162.79%, respectively, with growth rates much higher than global levels. Conclusion From 1990 to 2019, the disease burden of common malignant tumors in the reproductive system of Chinese women showed an overall upward trend. It is necessary to prioritize the prevention and control of menopausal and postmenopausal women, increase publicity and education for the three types of cancer, advocate healthy lifestyles, actively carry out cancer screening, and increase people's willingness to receive vaccines, in order to reduce the disease burden of cervical cancer, uterine cancer, and ovarian cancer in Chinese women.

Abstract: Background  Esophageal cancer(EC) is one of the most aggressive gastrointestinal tumors.Advanced esophageal carcinoma is mainly associated with dysphagia. Most elderly patients with severe obstruction esophageal carcinomacannot tolerate anesthesia and invasive treatment due to comorbidities, while the failure to improve dysphagia in the short term will seriously affect the nutritional status, life quality and prognosis of patients. Objective  To explore the safety and efficacy of image-guided photodynamic therapy (IGPDT) under local anesthesia for short-term improvement of obstruction and nutritional status in elderly patients with severe obstruction esophageal carcinoma. Methods  A total of 24 elderly patients with severe obstruction esophageal carcinoma admitted to Hebei General Hospital from March 2020 to December 2021 were selected for IGPDT in the prospective,single-arm,self-control study. The upper boundary of the lesion was located by endoscopyand marked with metal tissue clips, the lower boundary of the lesion was located by CT and esophagography before treatment. During the treatment, the fiber of laser treatment was delivered to the lesion site under the guidance of X-ray fluoroscopy during treatment. The Stooler dysphagia score was evaluated before,1 week and 1 month after operation. The nutritional status of patients was evaluated by nutritional risk screening 2002 score(NRS 2002),hemoglobin,BMI,albumin and prealbumin before and 2 months after operation. The swallowing quality of life scale(SWAL-QOL) was used to evaluate the quality of life in patients. Results  All patients achieved partial response(PR) at 1 month postoperative efficacy evaluation. The Stooler dysphagia scores at 1 week and 1 month after IGPDT were significantly lower than that before operation(P<0.001). BMI,albumin and prealbumin at 2 months after operation were significantly increased than that before operation(P<0.05). The total score and scores of different dimensions in SWAL-QOL at 2 months after operation were significantly higher than those before operation, including psychological burden, appetite, eating time, fear of eating and dysphagia(P<0.05). 92%(22/24) of the patients presented with grade 1-2 mild fever, and most of them could be relieved on their own. 83%(20/24) of the patients presented with grade 1-2 pain at the site of operation at 1-2 days after surgery but could be relieved by themselves. No esophageal fistula occurred in all patients during the operation, and no serious adverse effects of grade 3 or above occurred. Conclusion IGPDT has the advantages of easy operation, excellent efficacy and safety, and short-term improvement of nutritional status in patients, which can be used as a novel photodynamic therapy for patients with severe obstruction esophageal carcinoma cannot be passed by endoscopy.

  • Research progress of Helicobacter pylori related extra-gastric diseases

Subjects: Medicine, Pharmacy >> Preclinical Medicine submitted time 2023-05-27

Abstract: Helicobacter pylori (Hp) is a common gram-negative bacterium in gastrointestinal tract infection. It mainly exists on the surface of gastric epithelial cells and mucus, and is related to gastric ulcer, gastric cancer and gastric mucosa associated lymphoma. Some studies have shown that helicobacter pylori may induce or aggravate some extra-gastric diseases. Recently, other studies have reported that helicobacter pylori is associated with novel coronavirus pneumonia; It indirectly or directly participates in the occurrence and development of diseases by stimulating the body to produce inflammatory factors or cross immune response. In addition, Helicobacter pylori can also enter the Candida, release toxins to participate in extra-gastric diseases and evade the immune system and drug effects. This paper summarizes the research reports on helicobacter pylori related extra-gastric diseases abroad in recent years, aiming to arouse the attention of clinical workers to helicobacter pylori related extra-gastric diseases, popularize their relevant knowledge, and avoid the aggravation of helicobacter pylori or other diseases.
