  • Construction of a Service Quality Assessment Indicator System for Home-based Rehabilitation APP for Heart Failure Patients

    Abstract: Background Nowadays,cardiac rehabilitation is mostly transferred from the hospital to a home environment. Mobile health applications assist patients in bridging the support gap from hospital to home environment,which encourages home-based recovery processes. Unfortunately,numerous quality difficulties accompany its promise,and the service quality of cardiac rehabilitation applications must be 〝confirmed〞 as soon as possible. Objective To develop a quality assessment indicator system for home-based rehabilitation APP services for heart failure patients,and to provide a model for standardizing and increasing the quality of mobile health APP. Methods This study was conducted from July 2022 to February 2023,using semi-structured interviews,literature review,the expert consultation method and analytic hierarchy process(AHP) to determine the content of indicator system and the weight for each index. Results 2 rounds of expert consultations resulted in the effective response rates of 100%,with the authority coefficients of 0.867,and the coordination coefficients of 0.128- 0.154 and 0.151-0.197 respectively(P<0.001). As a result,an assessment indicator system for home-based rehabilitation APP services for heart failure patients was developed from three dimensions:software usability,medical professionalism,and behavioral science,with 7 primary indicators,24 secondary indicators,and 51 tertiary indicators. Conclusion The developedquality assessment indicator system for home-based rehabilitation APP services for heart failure patients is comprehensive and scientific,and can be useful for assessing and measuring the quality of heart rehabilitation APP services,as well as facilitating the benign development of similar APP services.

  • Reasons for Refusal of Exercise Rehabilitation in Patients after Radiofrequency Ablation of Atrial Fibrillation in the Context Digital Medical Care:A Qualitative Research

    Abstract: Background  Telerehabilitation based on digital medical care can efficiently improve the health status of patients after radiofrequency ablation of atrial fibrillation. However,the current participation rate in telerehabilitation is low. Objective  To analyse the reasons for refusal of exercise rehabilitation in patients after radiofrequency ablation of atrial fibrillation in the context digital medical care based on the theory of leisure constraints. Methods  Patients after radiofrequency ablation of atrial fibrillation in the inpatient department or outpatient clinic of cardiology at a tertiary-level hospital in Jiangsu Province from July 2022 to September 2022 were selected as the study subjects by using the purposive sampling method. The phenomenological approach was adopted to collect data from patients who refused exercise telerehabilitation after radiofrequency ablation through semi-structured interviews,and Colaizzi analysis was used to summarize the reasons. Results  A total of 14 patients were finally included in this study. Three themes including self-limiting factors,interpersonal limiting factors,and structural limiting factors,and twelve sub-themes were extracted,namely,low level of digital literacy,negative illness perception,psychological distress caused by disease,digital medical trust crisis,deep-rooted personal exercise habits,alienation sense from rehabilitation team,insufficient social network establishment,economic burden related to equipment acquisition,harsh climate,low rehabilitation service capacity in primary care,constraints of available time by role pressure,and poor applicability of wearable devices. Conclusions  The reasons for refusal to exercise telerehabilitation include lack of literacy and trust in digital medical care,high level of illness perception and psychological distress,poor exercise habits,rehabilitation team and peer alienation,economic burden,harsh climate,lack of capacity and personal time for rehabilitation service capacity in primary care,and inadequate applicability of existing wearable devices. It is necessary to build an effective support system and social network in exercise telerehabilitation on the basis of strengthening digital literacy and promoting mental recovery of patients in the future,strengthen the construction of the rehabilitation team to meet the requirements of digital medical care context,improve the treatment-rehabilitation-long-term care service chain centered on smart health and the reinforcement of rehabilitation service configuration in primary care,which is helpful to promote the participation of patients after radiofrequency ablation of atrial fibrillation in exercise telerehabilitation.

    Abstract:【目的】为了防止人才技术及经验流失,进一步规范统一作业操作流程极为重要。【方法】文章分析了科技期刊出版流程中存在的问题,梳理了出版流程,明确了数道专业工序,结合出版行业相关标准规范,将专业技术人员多年技术经验转化为出版指南,完成了科技期刊出版标准作业指导书的制定。【结果】通过应用科技期刊出版标准作业指导书,提升了科技期刊的出版质量和专业技术人员的工作效率,还增加了出版单位的技术资产,降低了出版单位的成本。【结论】文章为推行标准作业指导书体系,提升出版单位的运行效率提供了一种新思路。

    Abstract:“MCN+教育”是当前的社会热点内容,既契合了两会讨论的线上教育话题,又符合经济、技术及社会的发展要求。但由于用户增多,及平台的成熟推广,创作者与用户的界限模糊,“MCN+教育”出现了一定的困难与挑战,知识内容良莠不齐、同质化严重等问题亟待解决。本研究对B站MCN展开分析发现,优质知识内容生态与创作者特征以及MCN资源整合相关度高,而当前教育知识内容推广的现实与MCN+教育创作优质内容,形成良性产业氛围的目的存在一定的差距。由此,项目提出“MCN+教育”以人才培养为核心的优质内容创作的计划,以完善教育类内容生态扶持UP主的体系。

  • Collaborative Innovation and Guarantee Mechanism Construction of Libraries and Information Agencies and Think Tanks from the Perspective of Cooperation Alliance

    Abstract: [Purpose/significance] This paper aims to establish the cooperative innovation mechanism and guarantee mechanism of libraries and information agencies and think tanks, achieving resource integration and collaborative innovation of them.[Method/process] This article uses literature review and theoretical research methods to elaborate the feasibilities, necessities and values of collaborative innovation of libraries and information agencies and think tanks. Based on this, the paper establishes the cooperative innovation mechanism from ways of thinking, organization structure, resources and service content for the libraries and information agencies and think tanks, and then establishes the guarantee mechanism from six aspects:legal policies, management systems, funds, talents, technologies, and information resources.[Result/conclusion] This paper constructs the cooperative innovation mechanism and guarantee mechanism of libraries and information agencies and think-tanks from the perspective of cooperation, and provides a platform for resources integration, collaborative development, function expansion, society service, and integration of "politics, production, learning, research and practice".

  • Study on the mechanism for Danggui anti-carbon ion radiation based on network pharmacology

    Abstract: [Objective] Network pharmacology technology was used to explore the mechanism of Danggui anti-carbon radiation.
    [Methods] Danggui active ingredients and Targets of Danggui anti-carbon ion radiation were obtained through related databases. Target biological networks were constructed, bioinformatics analysis of targets were conducted. Molecular docking was performed to verify the results of network pharmacology.
    [Results] Nine active ingredients of Danggui participate in signaling pathways such as PI3K-AKT and MAPK through ninety-eight targets, regulate molecular functions such as DNA binding, kinase binding, and DNA damage response and other processes. Molecular docking verifies the reliability of network pharmacological results.
    [Limitations] The results of network pharmacology were only verified by molecular docking.

  • Application of home-based resistance training in cardiac rehabilitation for patients with coronary heart disease

    Abstract: Objective To investigate the effect of home-based resistance training in cardiac rehabilitation for patients with coronary heart disease (CHD). Methods Totally 122 CHD patients requiring home-based cardiac rehabilitation were divided into the control group (n=61) and the observation group according the sequence of hospital admission. All patients received community care and health education by telephone follow-up, and patients in the observation group were given health education on cardiac rehabilitation and home-based resistance training. The physical function, motor ability, quality of life and cardiac parameters of patients were measured. Results After a three-month intervention, The scores of Chinese Mini Physical Performance Test(CM-PPT) and Chinese Questionnaire of Quality of Life in Patients with Cardiovascular Diseases(CQQC) in the observation group were higher than those in the control group (P<0.01). Patients in the observation group achieved higher outcomes of 30-second Arm Curl Test (30-ACT), longer distance of 6-Minute Walking Test and shorter time of 10x Sit-To-Stand Test (STS10) compared with those in the control group after intervention (P<0.05). Higher level of Cardiac output (C0) and Left Ventricular Ejection Fraction(LVEF) and lower level of Left Ventricular End Diastolic Volume (LVEDV) and Left Ventricular End Systolic Volume(LVESV) were reported in the observation group than those in the control group(P<0.05). Conclusion Application of health education on cardiac rehabilitation and home-based resistance training could improve the physical function, motor ability, cardiac function and quality of life in CHD patients.

  • Analysis of Research Hot Spots of Multiple Chronic Conditions in the Elderly in 2010-2021

    Abstract: Background With the acceleration of population aging,multiple chronic diseases have become common in the elderly,and the research on multiple chronic diseases in the elderly is also increasing. However,few studies have been conducted to analyze its progress and hot spots. Objective To analyze the hot spots of multiple chronic diseases in the elderly at home and abroad,revealing the hot spots in frontiers research,so as to provide a reference for related researchers to track cutting-edge information. Methods The literatures related to multiple chronic diseases intheelderly were retrieved from Web of Science,Scopus,CNKI,Wanfang Knowledge Service Platform,VIP,PubMed,Chinese Medical Association Journal Fulltext database,and APA-PsycINFO database of the American Psychological Association with the retrieval time of 2010-2021, based on text mining technology and bibliometrics method,etc. CiteSpace 6.1.3,PASW 18,BICOMB 2.04 and other software were used to analyze and visualize the trends in number of papers published,sources,authors,institutions,keywords and other aspects of literature publication. Strategic coordinate maps were drawn to analyze research hotspots in the field. Results Finally,9 392 literatures related to multiple chronic diseases in the elderly were involved,including 5 776 literatures in foreign language and 3 616 literatures in Chinese. The numbers of literatures related to multiple chronic diseases in the elderly both in foreign language and Chinese have increased exponentially in the past decade, with the annual growth rate of 13.27% in Chinese literature and 15.84% in foreign literature,suggesting the development stage of multiple chronic diseases in the elderly. Five mainstream research hotspots have been identified in both Chinese and foreign literatures. There are more literatures related to multiple chronic disease such as chronic obstructive pulmonary disease,diabetes mellitus,hypertension,and respiratory diseases. The foreign literature shave focused more on the psychiatric comorbidity,followed by somatic comorbidities such as diabetes mellitus,hypertension,and heart failure. Conclusion The analysis results shows that the field of multiple chronic diseases in the elderly is in a rapid development stage. Analysis of foreign literature shows that psychiatric comorbidity is a relatively hot topic but not mature enough,research results related to multiple chronic diseases in the elderly are abundant and diversified,but the research depth is in sufficient. Domestic literature analysis shows that chronic obstructive pulmonary disease combined with respiratory diseases in the elderly is at the core of the field,and there is a lack of studies related to psychiatric comorbidity such as depression and mutiple chronic diseases such as health situation,nursing and health care,which may become the main direction of research development in the future.

  • 冻融干湿交替下灰绿板岩矿质元素释放特征及释放模型

    Abstract:针对宁夏中部干旱带压砂地土壤肥力下降、灰绿板岩矿质元素淋溶释放规律不明等问题,通过室内模拟灰绿板岩冻融干湿循环交替试验,采用修正的Elovich方程、抛物线方程、双常数速率方程、一级动力学方程对各矿质元素的累积释放曲线进行拟合,研究不同粒径板岩在冻融干湿循环作用下的释放动力学特征及最优动力学方程。结果表明:两种粒径灰绿板岩淋溶液中矿质元素淋溶总量随循环次数增大而增大,1 cm粒径灰绿板岩矿质元素累积淋溶总量、淋溶速率在不同循环次数下均大于3 cm粒径。灰绿板岩矿质元素淋溶释放是多因素共同控制的物理、化学过程,其释放过程可大致分为快速反应阶段和反应趋于平衡阶段;压砂地土壤中Ca、K、Mg、P元素的供给量运用修正的Elovich方程预测结果较好,抛物线方程更适用于描述S元素的释放规律。研究结果可为宁夏中部干旱带旱作农田土壤肥力调控提供决策参考。

  • Research progress on cost-effectiveness of atrial fibrillation screening

    Abstract: Atrial fibrillation is one of the most common arrhythmias in clinical practice. The onset of asymptomatic atrial fibrillation is insidious and the awareness rate is low, which can easily lead to adverse outcomes and bring a serious burden. Disease screening should precede the prevention of complications, and the screening and scientific management of asymptomatic atrial fibrillation are crucial. This article systematically reviews the screening of atrial fibrillation in recent management guidelines, and from the perspective of health economics, analyzes the impact of different screening populations, screening equipment, and monitoring time on the cost-effectiveness of atrial fibrillation screening strategies. At the same time, the role of screening participation rate and anticoagulation treatment compliance on economy was also clarified, in order to guide clinical practice.

  • 白菜CesA 基因家族鉴定及表达模式分析

    Abstract: Cellulose is the main component of plant cell walls. It is involved not only in cell morphology and development, but also in various cellular signal transduction pathways, thus affecting plant growth and development. Cellulose synthase is the main enzyme of cellulose synthesis in plants. In order to explore the role of CesA gene family in cellulose synthesis and development of Brassica rapa, we identified the members of CesA genes in B. rapa genome via bioinformatic method. The results were as follows:(1) We identified 16 BrCesA genes from 10 B. rapa chromasomes with pl range from 4.76-9.12, molecular weight range from 17.76-122.67 kD and amino acid length range from 153-1 089 aa. (2) With an expectation of Bra036008, which located in scaffold, the rest of 15 BrCesA genes unevenly distributed in 7 chromosomes. (3) Most of CesA contained 4-14 exons and 1-11 conserved motifs. Sequence alignment showed that all CesAs contained DDD-QXXRW domain. (4) Subcellular location analysis presents a typical N terminal, C terminal and transmembrane structure and the protein showed plasma membrane location. (5) Tissue specific analysis by qRT-PCR showed relative highly expression in stem of B. rapa seedlings. (6) When seedling treated with 150 mM·L-1 NaCl, the expression of Bra01069 significantly increased after 6 h. At present, although B. rapa reference genome sequencing research has advanced to the new version v3.0, the CesA gene family has not been thoroughly identified and analyzed in B. rapa, and the functions of many genes have not yet been clarified. In this study, bioinformatics methods were used to take the whole genome sequence of B. rapa as the research field, and the whole genome identification of the CesA gene family of B. rapa can not only lay the foundation for the subsequent in-depth study of the function of CesA gene, but also lay the foundation for the growth and development of B. rapa.

  • 三种山茶属金花茶组植物花朵类黄酮成分研究

    Abstract:以山茶属金花茶组金花茶、凹脉金花茶和崇左金花茶为材料,利用超高效液相色谱一四极杆一飞行时间质谱联用技术定性定量分析其花朵中类黄酮成分与含量。结果表明,三种植物中检测到15种类黄酮,其中天竺葵素一3-0-葡萄糖昔、木犀草素、木犀草素一7-O一芸香糖昔、棚皮素一3,7-0一二葡萄糖昔、芸香袖皮昔、圣草素和染料木昔为金花茶组首次发现;棚皮素一3-0-葡萄糖昔、棚皮素一7-0-葡萄糖昔、棚皮素一3-O一芸香糖昔和山奈酚-3-0-葡萄糖昔为凹脉金花茶和崇左金花茶中首次发现。儿茶素、表儿茶素、棚皮素一3-0-葡萄糖昔、棚皮素一7-0-葡萄糖昔、棚皮素一3-O一芸香糖昔和山奈酚-3-0-葡萄糖昔为三个物种主体成分;天竺葵素-3-0-葡萄糖昔为金花茶特有,棚皮素一3,7-0一二葡萄糖昔为崇左金花茶特有;木犀草素一7-O一芸香糖昔主要存在于金花茶和崇左金花茶;木犀草素主要存在于凹脉金花茶和崇左金花茶。类黄酮类型主要为儿茶素类、棚皮素类、木犀草素类和山蔡酚类;崇左金花茶中棚皮素类、木犀草素类及类黄酮总量远高于金花茶和凹脉金花茶,凹脉金花茶和崇左金花茶儿茶素类高于金花茶,金花茶和崇左金花茶山蔡酚类高于凹脉金花茶。

  • 连续 5 年脲原体属和人型支原体体外药物敏感性回顾性分析及喹诺酮类耐药机制研究

    Abstract: Objective To analyze the culture and in vitro antimicrobial susceptibility of Ureaplasma and Mycoplasma hominis in Peking Union Medical College Hospital for 5 consecutive years and mechanisms responsible for fluoroquinolone resistance.Method The study reviewed all of the Ureaplasma and Mycoplasma hominis cases detected by in vitro culture from September 2012 to April 2017 in Peking Union Medical College Hospital, with the information of patients and species, its charactoristics of antimicrobial susceptibility testing were also analyzed. Meanwhile, mutations in DNA gyrase (GyrA/GyrB) and topoisomerase IV (ParC/ParE) which were related to fluoroquinolone resistance were detected in this study.Results The in vitro sensitivity of Ureaplasma mixed with Mycoplasma hominis was significantly lower than that of Ureaplasma or Mycoplasma hominis alone. Except for macrolides, Ureaplasma was less susceptible to quinolones, tetracycline, josamycin, and primycin than that of Mycoplasma hominis. In addition, the susceptibility of Ureaplasma to azithromycin, erythromycin, clarithromycin, ofloxacin in female patients was lower than that in male patients. Ureaplasma parvum were more susceptible than Ureaplasma urealyticum to most antibiotics, especially for tetracycline (25.8% discrepancy), p<0.05. Moreover, twenty-one mutations from sequences of GyrA, GyrB, ParC and ParE were determined. Mutation in ParC, with a S83L substitution being most frequent, 96.22%; A136T substitution in ParC, R448K substitution in ParE, and L176Fof GyrA combined with S83L in ParC was also detected. This study also found Six novel mutations, L540F, R718W, Q767E, S789N, M828I and I831T amino acid substitutions in ParC protein.

  • 如皋市新生儿窒息死亡率阶段性比较分析

    Abstract:摘要: [目的] 评估综合干预措施防控新生儿窒息死亡率的效果。 [方法] 对2011-2016年与1991-2010年新生儿窒息死亡资料进行回顾性分析与阶段性比较。 [结果] 2011-2016年与1991-2010年比较,新生儿窒息死亡率差异有统计学意义(X2=36.935,P=0.000),二级和三级医院出生的新生儿窒息死亡率差异无统计学意义(X2=1.371,P=0.242),0天、1天以上新生儿窒息死亡构成比差异无统计学意义(X2=0.336,P=0.562),不同出生地点的构成比差异有统计学意义(X2=40.468,P=0.000),不同死亡地点的构成比差异有统计学意义(X2=7.505,P=0.017),1991-2010年新生儿窒息平均增长量为0.07/10万,2011-2016年为-1.64/10万。 [结论] 如皋市新生儿窒息死亡率增长的现象得到扭转,综合干预措施发挥了一定的作用。建议继续加强复苏培训与复苏后救治体系建设。 关键词:新生儿窒息;新生儿复苏;婴儿死亡率;再培训

  • 婴儿死亡率下降的理想历程分析

    Abstract:[目的] 探索婴儿死亡率下降的历程。 [方法] 以日本和中国香港婴儿死亡率动态数列为参照系统,借鉴研究经济现象的方法,采用平均增长量、平均发展速度、平均增长速度描述分析婴儿死亡率的变化规律,分析下降历程。再通过婴儿死亡率随人均GDP变化轨迹进行验证,进一步分析191个WHO会员国2000-2013年下降规律与参照系统的符合率。按参照系统婴儿死亡率下降特征判别现阶段中国大陆婴儿死亡率所处的位置,预测未来下降历程。 [结果] 参照系统婴儿死亡率下降大致经历快速下降、缓慢下降、低水平持续状态三个阶段,婴儿死亡率随人均GDP变化轨迹与时间序列的规律相一致,191个国家下降规律与参照系统总体符合率为59.68%,快速下降、缓慢下降、低水平持续状态符合率分别为98.57%、21.62%、61.70%。中国大陆地区婴儿死亡率仍处于快速下降阶段,正向缓慢下降阶段过渡,“十四五”后期将进入低水平持续状态;城市婴儿死亡率处于缓慢下降阶段,“十二五”后期将向低水平持续状态过渡。 [结论] 婴儿死亡率下降存在着内在的变化规律,理想的下降历程值得发展中国家因地制宜地学习借鉴。在不同的发展阶段应制定、落实更有针对性的防控策略与措施。

    Abstract:可扩展计算机系统在各个领域得到了越来越广泛的应用,这些应用往往具有可扩展的需求,而这些可扩展应用的特征却有很大差异。过去20 年间,用于可扩展应用的系统平台层出不穷,这些平台的优势也各不相同,评价一类应用与某种系统平台相适应的程度成为用户关注的一个关键问题。本文对可扩展应用及可扩展系统进行了综述与分析,并提出了一些评价应用与系统平台匹配程度的参考因素;同时,本文还对近来业界提出的一些热点新名词进行了解释与分析,比较了它们之间的异同。本文的目的是帮助人们深入理解可扩展应用与可扩展系统的特征,帮助用户选择合适的平台以提高应用的效率和资源的利用率,同时激发科研人员进一步探索适应应用新需求的系统平台技术。
