  • A Qualitative Study of Community-Dwelling Elderly People' Experience of Multimorbidity

    Abstract: Background In the face of the serious challenges posed by the high prevalence of chronic diseases among the elderly and the coexistence of multiple chronic diseases,the health problems of community-dwelling elderly people' with multimorbidity require urgent attention. The comorbidity management model is being explored and practiced. Disease experience,as self-reported health information from patients,is expected to become a key component of the patient-centered community multimorbidity management model. Objective This study aimed to gain a deeper understanding of the real disease experience of community-dwelling elderly people with multimorbidity and provide a basis for personalized health management and supportive strategies of the group. Methods This study used purposive sampling and maximum difference sampling to select eligible interviewees from four communities under the jurisdiction of a community health service center in Zhengzhou City from August to September in 2022. Based on the principle of information saturation,20 subjects were ultimately included. Based on the research objective,literature review,and expert consultation results,the preliminary interview outline was formulated, and the final interview outline was determined after interviewing 2 patients. Semi-structured interviews were conducted with the study participants,guided by the phenomenological research methodology. Golaizzi analysis was used to analyse the data. Results Four themes and eleven sub-themes had been extracted from this study,including mainly long-standing symptomatic distress(physical dysfunction,fatigue,sleep disorders,and pain),negative psychological experiences (feelings of helplessness and worry about future life,reduced sense of social value and self-identity,and loneliness with altered social activities), positive mindset of coexisting harmoniously with multimorbidity(being grateful for being satisfied,and positively adapting), and aspirations in times of adversity(hoping for improvement in functioning,and seeking multi-directional social support). Conclusion Physical dysfunction,sleep disturbances,fatigue,and pain are the most concerning health disturbances for community-dwelling elderly people with multimorbidity. The symptom management and multiple social support of the multimorbidity need to be strengthened. In the future,it is expected that multidimensional data would be used for dynamic health assessment to explore the common and individual patterns of multimorbidity,and to achieve precise management and intervention of elderly multimorbidity in the community.

  • Perspectives on the Application of Neural Oscillations in the Diagnosis and Treatment of Consciousness Disorders

    Abstract: The diagnosis and treatment of disorder of consciousness(DOC)is still challenging. One of the neural mechanisms of DOC is the destruction of synchronization and connectivity of neural oscillation. This article will review the progress of neural oscillation in the diagnosis and treatment of DOC from three aspects:theoretical basis,application progress,and existing deficiencies of neural oscillations applied to DOC. Neural oscillation is the periodic and synchronous activity of neurons under certain conditions. There are obvious differences in the spectrum characteristics,phase synchronization,and response to neural regulation of neural oscillations among different severities of DOC. The analysis of neural oscillation based on electroencephalography(EEG)reveals the information transmission and processing ability at different levels of consciousness,which is helpful to distinguish different degrees of DOC more accurately and achieve the purpose of DOC wakening treatment by regulating neuro oscillation. Despite there being some problems like insufficient mechanism studies and slow application transformation,neural oscillation has shown great application potential in the diagnosis and treatment of DOC.

  • Dynamic characteristics of DOC release from the mixed leaf litter decomposition of coniferous and broad-leaved tree spieces

    Abstract: In order to adjust the structure of inefficient Pinus massoniana forest and explore the dynamic changes for DOC during the mixed decomposition process of P. massoniana and native broad-leaved tree species. Our research focuses on the leaf litter of P. massoniana, Cinnamomum camphora and Toona sinensi, and combine them into the 15 treatments (3 single tree treatments + 12 mixed treatments) according to different tree species combinations and mass ratios, and then conducted field decomposition experiments to further explore the optimal combination for DOC release. The results showed that: (1) DOC content of Pinus massoniana and most of mixed treatments (except for PT64) significantly increased during the early stages of decomposition (0-6 months), leading to carbon enrichment. The DOC content of leaf litter decreased with the extension of decomposition time, and also exhibit carbon enrichment in small degree during the middle to late stages of decomposition (12-18 months) or the late stages of decomposition (18-24 months). Mixed leaf litter showed that the higher the proportion of broad leaves, the lower the DOC content in the later stage. (2) The antagonistic effect of DOC release rate in the early stage of decomposition (0-6 months) was strong (58.33%), and its synergistic effect gradually strengthened (91.67%) after 6-18 months. But the synergistic effect of leaf litter weakened (66.67%) during the late stage of decomposition (18-24 months). Among all mixed treatments, PT64 exhibited synergistic effects throughout the entire decomposition period, followed by PT73, PCT622 and PCT613(3/4). (3) Partial Least-Squares Regression (PLS) showed that N content, P content, lignin content, condensed tannin content, C/N, C/N, lignin/N, and lignin/P were important factors affecting the release of DOC from litter in this study area. Overall, the DOC release from mixed P. massoniana and broad-leaved litter was affected by tree species, mixing ratio and decomposition time. Among all of the mixtures, PT64, PT73, PCT622 and PCT613 showed strong synergistic effects for DOC release.

    Abstract:自从互联网诞生开始,科学技术便大踏步地持续向前发展,促使了全媒体时代的应运而生。但也正因全媒体时代的到来,传统媒体大受冲击,各行各业的报刊因此受到了不同程度的影响,其中也包括曾在少儿教育领域占有一席之地的少儿报刊。因此,传统少儿报刊要想在媒体融合背景下的今天,重新占据报刊市场的一席之地,就不得不顺应时代的发展,将新媒体的理念融入报刊创办宗旨内,以全新的面貌走上一条优化产业结构,革新创收模式的转型之路。基于此,本文将以少儿报刊为例,结合新媒体融合概述、新媒体时代信息发展的趋势、新媒体融合背景下的少儿报刊现状以及媒体融合背景下少儿报刊编辑应有的素养,来进一步探究媒体融合背景下少儿报刊编辑转型的具体路径,以期为当今时代的少儿纸媒转型提供参考。

  • Knowledge Vein and Forefront Outlook of Information Encounter Behavioral Research

    Abstract: [Purpose/significance] An in-depth systematic analysis of information encounter behavior can promote academic innovation in this field and application innovation in related information services.[Method/process] Using CNKI and Web of Science as data sources, a systematic review method was used to analyze the most representative 41 literatures in the field. In the research section of the knowledge vein, this paper systematically analyzed and summarized the theoretical models, research methods and influencing factors. In the forefront perspective, the process of encountering information is broken down into three stages:before encounter, during encounter, and after encounter. Then, the paper summed up the study of encountering follow-up behavior.[Result/conclusion] Information encounter behavior is the result of the combined effects of information, individuals, technology, and the environment. The use of encounter information is currently a research hotspot. Research on information encounter based on the information search process chain, research based on the mobile Internet, research based on objective data, and cross-discipline research are hot topics that are feasible in the future.

  • Systematic Review of Online Health Information Help-Seeking Behaviour: Knowledge System, Factors and Frontier Analysis

    Abstract: [Purpose/significance] Empirical research is an important part of users' online health information help-seeking behaviour research, and its in-depth analysis can promote the application innovation and pattern innovation of online health information service. [Method/process] This paper takes CNKI and Web of Science as the data source to obtain the representative research documents. Systematic review method is used to comb the related empirical research literature of users' online health information help-seeking behaviour both at home and abroad. [Result/conclusion] Existing studies have initially formed a framework of knowledge systems. However, there are still many research gaps. The influencing factors mainly include internal factors (help-seeking items, help-seeker factors), external factors (information environment, social environment, aid provider factors), mediators (help-seeking demands, cognitive factors), and moderators (demographic factors). In the future, focusing on research foundation, mining influencing factors, paying attention to outcome variables, and promoting data-driven research will be the focus and hotspot.

  • Review of the Research on the Key Issues of Social Network Users' Intermittently Discontinuous Behavior

    Abstract: [Purpose/significance] Intermittently discontinuous behavior is an important part of the passive behavior of social network users. This paper makes an analysis of the research results at home and abroad, systematically sorts out its knowledge system and key issues of research in this field, and forecasts the future research trend accordingly, which can provide some reference for the follow-up academic research.[Method/process] With CNKI and Web of Science as the data sources, this paper obtains 54 representative research related to this fields through keywords retrieval, tracing retrieval and subject analysis, adopts the method of systematic review to integrate knowledge base through three key issues, including research topic, research theory and research method, and constructs the framework of normative research process based on this.[Result/conclusion] The study of users' intermittently discontinuous behavior in the context of social network is in the initial stage, and a large space remains for further study in terms of the subdivision degree of research topics, the richness of theory models and the diversity of research methods. Based on the interdisciplinary perspectives of information science, psychology, sociology, journalism and communication, it will be a future research trend to deeply explore the influencing factors and formation mechanism of social network users' intermittently discontinuous behaviour from the perspectives of two-way balance of promoting factors and inhibiting factors as well as comprehensive effect of external social environment and psychological and emotional experience.

  • Construction of a Staged Continuing Medical Education Model for General Practitioners with the Cooperative Framework between County-level Government and Medical Institutions and a Tertiary General Hospital

    Abstract: Continuing medication education for in-service general practitioners(GPs)is a key measure for strengthening the primary health care system,and the department of general medicine in a tertiary general hospital is an important institution undertaking the trainings for GPs. We introduced the development of a staged continuing medical education model for GPs initiated since 2020 by Deqing County Government with Health Bureau of Deqing County in collaboration with Department of General Practice,Sir Run Run Shaw Hospital,Zhejiang University School of Medicine and medical educations in Deqing,with a detailed description of the three stages of development,namely the formation of medical and educational framework of Sir Run Run Shaw Hospital in cooperation with the Deqing County Government and medical institutions in Deqing,the construction of “Sir Run Run Shaw Hospital-Deqing" regional GPs education community,and the implementation of trainings for GPs in primary care by the education community using a strategy of “hierarchical and progressive teaching,and cyclic strengthening”. The development of this model will offer a reference for the construction of a regional GP training model using the cooperative framework between county-level government and medical institutions and the department of general practice in a tertiary general hospital.

  • Efficiency and Prospects of Stratified Stepped Training for Regional General Practitioners in County-General Hospital Collaboration Framework

    Abstract: Background  Since October 2020,relying on the teaching resources and experience of Department of General Practice of Sir Run Run Shaw Hospital,a three-stage hierarchical progressive teaching and training of “teacher training-backbone training-continuous training” has been carried out for general practitioners in People's Hospital of Deqing County and 12 subordinate primary care institutions in Deqing County. Objective  To analyze the related efficiency of stratified stepped training for regional general practitioners from October 2020 to November 2021. Methods  The stage competencies of 14 senior faculty members were evaluated prior to participation in training(October 2020),at the end of rotation training of Department of General Practice of Sir Run Run Shaw Hospital(January 2021)and at the end of the 12-month training(October 2021);the competency evaluation covers five aspects:general practice occupational competence,general practice clinical competence,general practice skills,general practice teaching competence and general practice professional competence. The stage competencies of 28 core general practitioners were evaluated prior to participation in training(January 2021)and after completing the 12-month training(December 2021),the competency evaluation covers four aspects:general practice occupational competence,general practice clinical competence,general practice skills and general practice professional competence. The stage competencies of 82 junior echelon general practitioners were evaluated prior to participation in training (January 2021) and after completing the 12-month training(December 2021)the competency evaluation covers three aspects:general practice occupational competence,general practice clinical competence and general practice skills. Results  There were significant differences in general practice occupational competence among 14 senior faculty members(P<0.05);among them,the scores of general practice professional competence in the third month and the twelfth month of training were higher than those before training(P<0.05). There were significant differences in the scores of general practice clinical competence and general practice skills among the senior faculty members in different time points(P<0.05);the scores of general practice clinical competence and general practice skills in the third month and the twelfth month of training were higher than those before training(P<0.05). The scores of general practice teaching competence of senior faculty members in the twelfth month of training were higher than those in the third month of training(P<0.05). The scores of general practice professional competence,general practice clinical competence,general practice skills of 28 core general practitioners after training were higher than those before training(P<0.05). The scores of general practice professional competence,general practice clinical competence,general practice skills of 82 junior echelon general practitioners after training were higher than those before training (P<0.05). Conclusion  This study provides an orderly,robust and sustainable model of continuous training and ability improvement of regional general practitioners after practice with the leading and supporting of general hospitals,which can effectively enhance the competence level and improve job competency of regional general practitioners.

    Abstract: The design and implementation of county continuing professional development and training model for general practitioners is the core of the collaboration on continuing professional development of general practice between the department of general practice of Sir Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine and Deqing County of Zhejiang Province. This article introduces the design and implementation of the "hierarchical progressive" continuing professional development training model in the collaboration framework in detail,shares a information platform by virtue of high-quality resources of general practice department of the general hospital,develops the county-level professional development training model for general practitioners,providing reference for colleagues.

  • INFLUENCE OF PRE-DEFORMATION ON THE PRECIPITATION BEHAVIORS OFAl-Mg-Si-Cu ALLOY FOR AUTOMOTIVE APPLICATION

    Abstract: To reduce the weight of car body, Al-Mg-Si-Cu alloys have been widely used to produce outer body panels of automobiles due to their favorable high strength-to-weight ratio, corrosion resistance, weldability and good formability. Al- Mg- Si- Cu alloys belong to age- hardenable aluminium alloys, whose strength derives mainly from the matrix precipitation during aging treatments. However, their bake hardening response still need to be further improved to enhance their dent resistance. A novel thermo-mechanical treatment consisting of conventional pre-aging, pre-deformation and re-aging was developed to enhance the tensile properties and bake hardening increment of Al-Mg-Si-Cu alloys. In this work, the effect of pre-deformation on the precipitation behaviors of Al-Mg-Si-Cu alloy was studied by DSC, mechanical property measurement and TEM. The results show that, the GPzone dissolution rate decreases with increasing pre-deformation during the slow heating up process for the pre-aged alloy, the corresponding activation energies of 0, 5% and 15% pre-deformed alloy calculated by Avrami-Johnson-Mehl method are 137.1, 189.5 and 141.3 kJ/mol, respectively. If the pre-deformed alloys are directly bake hardened at 185 ℃ for 20 min, precipitation and bake hardening increment can be greatly improved by pre-deformation (the highest bake hardening increment is 160 MPa), but the bake hardening increment rate gradually decreases if the pre- deformation is above 10%. In addition, the GP zone dissolution rates of pre- deformed alloys after bake hardening treatment are much lower when the heat treatment temperatures are below one certain value, but if the treatment temperatures above it, the corresponding GP zone dissolution rates are higher than that of alloy without pre-deformation, finally, the activation energy changes from high value to low value even can be observed in the ln[(dY/dT)f/f(Y)]-1/T curve. For the β″ precipitation in the alloys, with increasing pre-deformation, its activation energy gradually decreases, corresponding gradually increase of precipitation rate.

  • INFLUENCE OF DIFFERENT THERMOMECHANICAL PROCESSES ON THEMECHANICAL PROPERTIES AND MICROSTRUCTURE OFAl-Mg-Si-Cu ALLOY SHEETS

    Abstract: To reduce the weight of car body, Al-Mg-Si-Cu alloys are becoming increasingly attractive as a candidate for material substitution used to produce the outer body panels of automobiles because of their favorable bake-hardening response. However, the formability still needs to be further improved compared to steels. In this work, the effect of the thermomechanical processing on the mechanical properties and microstructure of Al-Mg-Si-Cu alloy is studied through tensile test, OM, SEM and TEM observation, as well as EBSD characterization. The results reveal that there is almost no change in both strengths and strain-hardening exponent n of the sheets in T4P condition after different thermomechanical processing, but the average plasticity strain ratio r, planar anisotropy Δr and elongations in the three directions show obvious differences. The sheet undergone hot rolling, cold rolling, intermediate annealing, cold rolling and solution (processing II) has a better formability (r= 0.6187) and a weaker planar anisotropy than that subjected to hot rolling, intermediate annealing and then cold rolling before solution treatment (processing I). Although the particle stimulated nucleation (PSN) effect of processing I is remarkable during solution treatment, due to the appropriate controlling cold deformation and distribution of second-phase particles with different sizes in processing II, most of the recrystallization grains are equiaxial and the recrystallization texture is only consisted of CubeND, Cube and H with a low intensity. At last, according to the relationship between the microstructure and the thermomechanical processing, the microstructure evolution model during different thermomechanical processes is established.

  • 乌海市沿黄河两岸沙丘风沙流结构差异与冰面风沙特征

    Abstract:为了研究乌海市沿黄河两岸沙丘风沙流结构差异与冰面风沙特征,开展以黄河西岸-黄河冰面-黄河东岸为路径的风沙观测实验,通过对比分析黄河两岸沙丘风沙流结构、沙尘粒度特征,为黄河沙尘防控问题提供理论依据。研究表明: 两岸沙丘总输沙率均随着高度增加而减小,东岸输沙率垂直拟合优度较西岸差,风沙流比西岸更容易达到非饱和状态。 黄河冰面裂缝滞留沙尘粒径大于0.19 mm,较小颗粒则输移到东岸,东岸沙丘受矿采区粉尘影响,极细砂粒(0.05~0.1 mm)含量增加,极细砂平均含量东岸<西岸,沙尘平均粒径东岸<西岸。 沿黄河两岸沙丘与冰面沙尘的粒度参数峰度均为宽峰态,沙物质来源相似。

  • 基于分布式压缩感知和散列函数的数据融合隐私保护算法

    Abstract: Aiming at the security problems existing in the process of the data aggregation and transmission in crowd sensing networks, such as privacy leakage, incomplete information, data tampering. this paper proposed a data aggregation privacy protection algorithm based on distributed compressive sensing and hash function. Firstly, it used distributed compressive sensing method to sparsely observe the sensed data and remove the redundant data. Then, it utilized one-way hash function to obtain hash value of the observation data and filled the hash value with the unconstrained camouflage data into the observation data of sensory data to reach the aim of concealing the true sensor data. Finally, after extracting the camouflage data at the sink node, it obtained the hash value of the observation data again to verify the integrity of data. Simulation results show that the algorithm takes into account the privacy preserving and integrity protecting of data, and also can reduce the communication overhead greatly, which means the strong applicability and scalability in practical applications.

  • 群智感知网络个性化位置隐私保护算法

    Abstract: The existing privacy protection strategies in crowd sensing networks used the same privacy policies for all locations which overprotected led to the problems that some locations, others were not adequately protected and the sensing data was less accurate. In order to solve this problem, this paper proposed a location privacy protection algorithm to meet the users’ personalized privacy and security requirements. First, it mined users’ access duration, frequency and regularity at different locations according to the user's historical movement trajectory, which used to predict the social attributes of the locations to the users. Then, it combined the location’s social attributes and natural attributes to predict user-location sensitivity levels. Finally, considering the different privacy security requirements of users in different locations, it set a dynamic privacy decision scheme. Users with less sensitivity at each location were selected to participate in sensing tasks to ensure that users, in the safe privacy context, could contribute the accurate data with a higher level of spatiotemporal correlation. The simulation results show that the algorithm can improve the privacy protection level and the accuracy of the sensing data.

  • FoxM1 shRNA慢病毒载体构建及感染人前列腺癌细胞的稳定细胞株筛选

    Abstract: Objective To construct a recombinant lentiviral vector that co-express green fluorescent protein (GFP) and FoxM1 shRNA and establish a prostate cancer cell line with stable FoxM1 down-regulation. Methods Three interfering sequences targeting FoxM1 were designed and inserted into the lentiviral vector pHBLV-U6-ZsGreen-Puro. After identification by DNA sequencing, the lentiviral vectors carrying Foxm1 shRNA were packaged in 293 cells. The lentiviral particles were collected to infect human prostate cancer DU-145 cells, and the transfection efficiency was observed under fluorescence microscope; the interference efficiency was assessed using real-time PCR. DU-145 cells with stable FoxM1 down-regulation were screened with puromycin, and the expression level of FoxM1 was detected by Western blotting and the cell growth was observed using MTT assay. The stably transfected cells were examined for cell apoptosis and cell clone formation capacity with flow cytometry and colony formation assay. Results DNA sequencing demonstrated successful construction of the 3 FoxM1 shRNA lentivirus vectors. Real-time PCR showed a high interference efficiency of FoxM1 shRNA1 vector, which resulted in obvious down-regulation of FoxM1 in DU-145 cells. Western blotting showed that the expression of FoxM1 protein was decreased in FoxM1 shRNA1 lentivirus-transfected cells, which displayed a suppressed cell proliferation, increased apoptosis rate, and attenuated clonogenic ability. Conclusion We have successfully established a prostate cancer cell model with stable FoxM1 down-regulation, which shows lowered proliferative and clonogenic activities with increased cell apoptosis.

    Abstract:【目的】探究影响移动医疗APP 持续使用意愿的因素及其内在作用机理。【方法】从信息生态的研究视角出发, 分析信息、信息人、信息技术和信息环境4 类影响因素, 并基于期望确认模型提出研究假设构建研究模型。【结果】选取多个移动医疗APP 用户为实验者, 采用“日志追踪实验+调查问卷”的方式收集288 份有效数据并利用SmartPLS2.0 对模型进行检验。结果表明, 期望确认模型中的关系在移动医疗情境下均成立; 信息准确性和一致性、信息人的感知健康威胁、信息技术的易用性和响应性、信息环境的直接和间接网络外部性均会正向促进移动医疗APP 的期望确认和感知有用性; 而信息人的电子健康素养则会正向促进期望确认, 负向抑制感知有用性。【局限】实验样本数量有待扩充, 得出的结论有待进一步推广。【结论】用户的移动医疗APP 持续使用行为是信息、信息人、信息技术和信息环境共同作用的结果。

    Abstract:通过弯边变形、拉伸实验、OM、SEM、TEM观察等手段研究了不同浓度富铁相粒子对Al-Mg-Si-Cu系合金板材弯边性能的影响规律。结果表明,随着富铁相浓度增加,合金板材沿纵横向的弯边性能存在较大差异,不含富铁相粒子的合金板材沿两个方向弯边变形180°后外表面粗糙度增加,处于中等浓度(含0.2wt%Fe)时沿两个方向弯边变形后均具有优异的外表面质量,但是随着富铁相浓度的进一步增加(至0.5wt%Fe),合金板材沿两个方向弯边变形后均在外表面出现微裂纹。虽然富铁相粒子浓度增加对合金板材纵横向延伸率影响不大,但是拉伸断口、弯边和拉伸断裂后的侧表面组织均表明,不含富铁相的合金板材弯边变形后外表面粗糙度增加与剪切带组织密切相关,而含有较高浓度富铁相粒子的合金板材弯边变形后外表面出现的微裂纹与粗大富铁相粒子的尺寸、形态以及分布状态密切相关。此外,本文根据不同浓度富铁相粒子与合金弯边性能间的定量关系提出了合金弯边变形后外表面粗糙度增加和微裂纹形成的模型示意图。

  • 预变形对汽车用Al-Mg-Si-Cu系合金析出行为的影响

    Abstract:采用DSC、力学性能测试及TEM研究了预变形对Al-Mg-Si-Cu合金析出行为的影响。结果表明,预时效态合金在慢速率升温过程中,GP区溶解速率均随预变形量的增加而降低,利用Avrami-Johnson-Mehl方法求得经0%、5%和15%预变形后合金的GP区溶解激活能分别为137.1KJ/mol、189.5KJ/mol和141.3KJ/mol;若合金经不同预变形后直接进行185℃/20min烤漆硬化,预变形可有效促进沉淀相析出,提高烤漆硬化增量,最高达160MPa,不过预变形量大于10%时合金烤漆硬化增幅减缓;此外,经预变形处理后烤漆态合金的GP区溶解速率在一定温度下均较低,但高于某一温度后,相应的GP区溶解速率均高于未经预变形处理的,最终获得的ln[(dY/dT)φ/f(Y)]-1/T曲线甚至会出现高激活能向低激活能转化现象;不过随预变形量增加,β〃相析出激活能不断降低,析出速率不断增加,相应的TEM组织观察结果与此非常吻合。
